A Bid bond or certified check equal to 10% of the bid must accompany each bid. All bids and bonds must be on blank forms provided for that purpose, and the amount of the bid shall be stated in writing and in figures. Bids should be submitted in a sealed envelope provided for that purpose, and branch of work (all inclusive single prime for this project) and reference number must be indicated on the outside of the envelope. The following forms must accompany each bid: Disclosure of Ownership (if applicable), "Affidavit of Noncollusion and/or Conflict of Interest. Blank forms for that purpose are provided with the bid package. There is a goal of 18% for the participation of Historically Underutilized Businesses (HUB) in this contract. The successful bidder will be required to provide a list of HUB firms participating in the work as part of the post bid submittal to MATC. The apprenticeship requirements for this project are as follows: Apprentices are mandatory from the four (4) of the following eight (8) trades: General Laborer; Carpenter; Concrete Finisher; Electrician; Painter; Sheet Metal Worker; Steamfitter; Temperature Control Installer. The successful bidder will be required to provide a list of Apprentice Categories from those listed above which will be utilized during this period.
