For those contractors who are not prequalified, application for prequalification will be submitted on DOT Form 420-010 (Standard Questionnaire and Financial Statement) and must be received by the Prequalification Analyst no less than fifteen days prior to the scheduled bid opening to receive consideration for issuance of a bidding proposal for that bid opening. All bid proposals shall be accompanied by a bid proposal deposit in cash, certified check, cashiers check or surety bond in an amount equal to five percent (5%) of the amount of such bid proposal.
