Site work, paving and outdoor lighting for a mixed-use development in Warsaw, Missouri. Completed plans call for site work for a sidewalk / parking lot; for paving for a sidewalk / parking lot; road / highway; and road / highway.
The proposed work includes: Grading and Embankment, Street Lighting, Sidewalks, Asphaltic Pavement, Optional Pavement Base Widening, Modified Cold Milling, Aggregate Base, Curb and Gutter, Curbing, Storm Sewer, Driveways, Pavement Markings, Small Block Retaining Wall, Fencing, Seed and Mulch and Signage. Special Needs: If you have special needs addressed by the Americans with Disabilities Act, please notify Randy Pogue, at (660) 438-5522, or through Missouri Relay System, TDD, at least five (5) working days prior to the bid opening you plan to attend. The wage rates applicable to this project have been predetermined as required by law and are set forth in the Request for Bid appendices. When Federal wage rates are applicable and included, this contract is subject to the "Work Hours Act of 1962", (P.L. 87-581: 76 Stat. 357) and implementing regulations. The City of Warsaw hereby notifies all bidders that it will affirmatively ensure that in any contract entered into pursuant to this advertisement, businesses owned and controlled by socially and economically disadvantaged individuals will be afforded full opportunity to submit bids in response to this invitation and will not be discriminated against on the grounds of race, color, religion, creed, sex, age, ancestry, or national origin in consideration for an award. The DBE Goal established for this project is 6 %. Bid Security will be required in accordance with the Instructions to Bidders. Bidder qualifications may be required in accordance with the Instructions to Bidders. Bids shall be submitted in a sealed envelope marked Bid for Roadway Improvements for Main Street, City of Warsaw, Missouri. Bids received after the designated closing time will be returned unopened. CONTRACTOR SHOULD READ AND BE FULLY FAMILIAR WITH ALL CONTRACT DOCUMENTS BEFORE SUBMITTING A BID. IN SUBMITTING A BID, THE BIDDER WARRANTS THAT IT HAS READ THE CONTRACT DOCUMENTS AND IS FULLY FAMILIAR THEREWITH AND THAT IT HAS VISITED THE SITE OF THE WORK TO FULLY INFORM ITSELF AS TO ALL EXISTING CONDITIONS AND LIMITATIONS AND SHALL INCLUDE IN ITS BID A SUM TO COVER THE COST OF ALL ITEMS OF THE WORK. OSHA TEN HOUR TRAINING REQUIREMENTS: Missouri Law, 292.675 RSMO, requires any awarded contractor and its subcontractor(s) to provide a ten-hour Occupational Safety and Health Administration (OSHA) Construction Safety Program (or a similar program approved by the Missouri Department of Labor and Industrial Relations as a qualified substitute) for their on-site employees (laborers, workmen, drivers, equipment operators, and craftsmen) who have not previously completed such a program and are directly engaged in actual construction of the improvement (or working at a nearby or adjacent facility used for construction of the improvement). The awarded contractor and its subcontractor(s) shall require all such employees to complete this ten-hour program, pursuant to 292.675 RSMO, unless they hold documentation on their prior completion of said program. Penalties, for Non-Compliance include contractor forfeiture to the Contracting Authority in the amount of $2,500, plus $100 per contractor and subcontractor employee for each calendar day such employee is employed beyond the elapsed time period for required program completion under 292.675 RSMO. The prospective participants must certify by submittal of EPA Form 5700-49, Certification Regarding Debarment, Suspension and Other Responsibility Matters, that, to the best of its knowledge and belief, it and its principles are not presently debarred, suspended, proposed for debarment, declared ineligible or voluntarily excluded from covered transactions by any federal department or agency. Should a bidder find defect; it shall bring such defects to the attention of City. Changes necessitated thereby shall be in the form of addenda issued by the Consulting Engineer. Contractor Questionnaire. Each prospective bidder, including a joint venture, shall file a contractor questionnaire on the form furnished by the Commission, which is available on MoDOTs website. The contractor questionnaire shall be furnished to the Commission as a separate document apart from any other document submitted. A bid will not be opened and read unless a fully responsive contractor questionnaire is on file with the Commission at least seven days prior to the time set for the opening of the bids. A new contractor questionnaire shall be filed annually, except the Commission reserves the right to request a contractor questionnaire from any contractor as of any date if the Commission has shown reason to believe that the contractor's experience data may have changed from that shown on the questionnaire on file. This document shall include a record of the bidder's experience data. The Commission will use this information as an aid to determine in each instance the lowest responsible bidder and nothing contained herein shall be construed as depriving the Commission of the Commissions discretion in the matter of determining the lowest responsible bidder. All bidders shall verify that they have considered all written addenda. Neither the City nor the Consulting Engineer shall be responsible for oral instructions. Any written addenda issued during the time of bidding shall be covered and included in the bid. There will be no clarifications or exceptions allowed on the Bid. Bids are for a total bid package, total contract price . The City of Warsaw, Missouri, reserves the right to accept or reject any and all bids and to waive any technicalities or irregularities therein. Bids may be modified or withdrawn by written request of the bidder received in the office of the City Clerk, prior to the time and date for bid opening. No bidder may withdraw its bid for a period of sixty (60) days from the date set for the opening thereof. ALL BIDDERS AGREE THAT REJECTION SHALL CREATE NO LIABILITY ON THE PART OF THE CITY BECAUSE OF SUCH REJECTION, AND THE FILING OF ANY BID IN RESPONSE TO THIS INVITATION SHALL CONSTITUTE AN AGREEMENT OF THE BIDDER TO THESE CONDITIONS. THE PROJECT WILL BE AWARDED TO THE LOWEST, RESPONSIVE, RESPONSIBLE BIDDER.
