The Housing Authority of the City of Milwaukee, Milwaukee, Wisconsin, will receive Quotations for furnishing all labor, materials, equipment, and services for performing all work necessary and incidental for Challenge Grant/Milwaukee Home Program Various Residential Rehabilitation Work Items at Various Addresses.Regardless of method of conveyance, quotations must be received by the due date and time in order to be considered. Technical questions regarding the scope of work and specifications/standards may be directed to Alison Daase of Travaux/Housing Authority, 414-209-1805, alison.daase@hacm.org. Prospective bidders may also contact Ms. Daase to arrange an inspection of the sites if desired. Administrative questions regarding bidding and contract requirements may be directed to David Flores of the Housing Authority 414-209-1802 or david.flores@hacm.org.Bidder shall complete the Pricing section (below) of this Summary and Quotation Form. Quotations shall be for a Total Lump Sum. However, Bidder shall also provide a breakdown by address. Bidder shall indicate if it is a certified Small Business Enterprise (SBE) as defined in Chapter 370 of the Milwaukee Code of Ordinances and as administered by the Office of Small Business Development of the City of Milwaukee. Bidder shall attach a list of all proposed subcontractors and their respective trades or roles, indicating what dollar amount or percentage of the total lump sum bid is attributable to each subcontractor. Bidder shall identify any proposed subcontractors that are certified SBEs. Bidder shall provide three (3) references of past construction projects. For each reference, Bidder shall indicate the client or owner, the scope of services, the dollar value of the project, and the date of project completion. For each client or owner, Bidder shall provide address, contact person, telephone number, and e-mail address. The Housing Authority is exempt from Wisconsin Use and Sales Tax and from Federal Excise Tax. Therefore, bidders shall not add said tax to their bids when bidding to the Owner. However, bidders shall include in their bids the taxes they will be required to pay directly as a consumer when obtaining materials, equipment, etc. to fulfill the project requirements should they be the successful bidder. Bidders are ultimately responsible for determining the impact of the State and Federal taxes on their bids. The Housing Authority reserves the right to: reject any and all bids, quotations, or proposals; waive informalities, technical defects, and minor irregularities; award by item or in the aggregate; accept the bid, quotation, or proposal (if any) deemed most advantageous to and in the best interests of the Housing Authority. A late modification of an otherwise successful bid, quotation, or proposal that makes its terms more favorable to the Housing Authority of the City of Milwaukee may be considered at any time it is received and may be accepted. Award will be based on price, contractor's availability to provide the specified goods and/or services when required, and contractor's expertise and past performance. This Request for Quotations for Services Pricing does not represent an award or commitment to purchase. Any actual award or purchase will be confirmed by an official contract (purchase order) issued by the Housing Authority.
