The District is not responsible for bids received late. Any bids received after the scheduled closing time will not be reviewed. Amphitheater Public Schools prefers that the Respondent include with their hard copy bid response a copy of their bid on either CD or USB/flash drive. (Note: This is to comply with any public records requests that the District may receive after bid award.) A completed W-9, the appropriate Contractor's license in good standing, and a Schedule of Values are also required with submission of a bid. Questions concerning this solicitation must be directed to Brenda Widugiris, Purchasing/Warehouse Manager in writing at bwidugiris@amphi.com and submitted no later than end of day Friday, April 19, 2019. An amendment with answers to all questions received by this date will be published on Tuesday, April 23, 2019 at the portal. The District is seeking a qualified Contractor to provide a boiler replacement as per the scope of services in this solicitation document for IRHS located at 2475 West Naranja Drive in Tucson, AZ 85742. Further information about the District is located at the District's portal. Contractor is to provide a bid for this project based on the information and document listed. The following document is attached to and incorporated in to this solicitation document: Specifications RFB 05-03-2019.pdf (NOTE: A detailed schedule of values must be included as an attachment to the Contractor's bid pricing sheet and submitted with Contractor's bid.) Bid responses shall include all labor, any additional materials, testing, etc. to provide a complete finished product, including the appropriate demolition or disposal of any fixtures and other equipment no longer in service and as directed by the District. Project Information: A. Project Identification: Ironwood Ridge High School Central Plant Boiler Replacement. 1. Project Location: 2475 West Naranja Drive, Oro Valle AZ 85742 B. Owner: Amphitheater Public Schools, 701 W. Wetmore Road, Tucson, Arizona 85705. 1. Owner's Representative: Kevin Chester. C. McCook Boiler & Pump 1. Work Restrictions, General: Comply with restrictions on construction operations. 1. Comply with limitations on use of public streets and with other requirements of authorities having jurisdiction. B. On-Site Work Hours: Limit work in the existing building to normal business working hours of 6:00 a.m. to 5:00 p.m., Monday through Friday, unless otherwise indicated. C. Existing Utility Interruptions: Do not interrupt utilities serving facilities occupied by Owner or others unless permitted under the following conditions and then only after providing temporary utility services according to requirements indicated: 1. Notify Owner not less than two days in advance of proposed utility interruptions. 2. Obtain Owner's written permission before proceeding with utility interruptions. D. Restricted Substances: Use of tobacco products and other controlled substances on Project site is not permitted. E. Construction can start July 1, 2019 and all work and punch list items need to completed by 10/15/19. Approved Alternatives: A. Any proposed alternatives must be submitted as a pre-bid question and e-mailed as instructed on page one of this solicitation document for consideration. Scope of Work: 1. Removal and disposal of (1) existing Rite Boiler model WG-600, 6,000,000 BTU Input. Include all rigging/hauling fees. 2. Delivery, Offloading, Rigging, and Placement of (1) Parker Boiler model #T5700 Industrial Packaged Hot Water Boiler, Direct Fired, with the following features and trim items: a. 5,700,000 BTU min Input requirement b. Natural Gas Fired, 14" W.C Max inlet pressure to gas train c. CSD-1 trim and Controls d. ASME "H" Stamp, with ASME safety relief valve. e. Barometric Damper, with gas spillage switch f. Indicator Lights g. Dry contact terminals for remote enable/disable h. Electric Modulation, fully modulating firing rate control i. Second Cabinet Door for service and access j. 115 Volt, 1 phase, 60 Hz k. UL Listed and National Board Registered 3. Gas Piping modifications to adapt to the new gas train on Parker T5700. 4. Hot water supply and return modification to adapt to the new boiler inlet and outlet connections. Existing electric-actuated control valve to remain in place on boiler inlet side. 5. Adapt exhaust stack to existing stack opening, single exhaust vent provided on new boiler. Cap additional unused flue vent/roof penetration. 6. Electrical work to power the new boiler, install new fused disconnect, and connect to emergency stop button for boiler shut down. 7. Pipe safety relief valve, full size, to nearest floor sink. 8. Install regulator vents and vent to atmosphere. 9. Provide factory startup service, owner training session, and OM manuals. 10. Provide boiler inspection by state-authorized third party and obtain State of AZ operating permit. 11. Provide two year parts/labor warranty. 12. Add Parker 'REMOTE VIEW' Lead/Lag System Controller and Remote Touch-Screen Display, Requires the following additional boiler trim/control items: Operating Temp control upgrade for integration with lead/lag panel, Type J thermocouple sensors on boiler inlet and outlet, additional terminals for the temperature sensors. Field wiring and connections required in order to connect boiler #1 to the Lead-Lag Panel. This will allow for future integration of the lead-lag system when the second boiler is replaced. No contract exists on the part of the District until a written purchase order is issued. Issuance of a purchase order will be considered sufficient notice of acceptance of offer. (NOTE: Funds may not presently be available for performance under the awarded contract beyond the current fiscal year. No legal liability on the part of the District for any payment may arise under the awarded contract beyond the current fiscal year until funds are made available for performance of the awarded contract. The District will make reasonable efforts to secure such funds.) The start date will be confirmed by the District and communicated to the Contractor upon issuing the Notice to Proceed. Liquidated damages will be assessed in the amount of $250.00 for each day the work is not complete after the agreed upon completion date. A 10% bid bond is required with the Contractor's bid response. Upon bid award, the successful Contractor shall: 1. Provide evidence of worker's compensation. 2. Provide vehicle and general liability insurance certificates in the amount of $2,000,000.00 with the District shown as an additional insured. 3. Provide payment and performance bonding in the amount of the bid award. 4. Provide Builder's Risk Insurance insuring the interest of Owner, Contractor(s) and Sub Contractors of all tiers including, but not limited to, coverage against fire, lightning, wind damage, hail, explosion, riot or civil commotions, aircraft and other vehicles and collapse The policy(ies) for such coverage shall be secured and maintained by Contractor in an amount equal to the Full Completed Value of the project. Any deductible amounts under the policies shall be the sole responsibility of the Contractor. The General Contractor's bid shall include the builder's risk premium on an amount equal to 100 percent of the base bid plus all add alternates. Builder's risk insurance shall remain in effect until the building or project is accepted by the District, and the District has been notified in writing, and approves that the insurance is no longer required.
