Site work for a water / sewer project in Whitehall, Ohio. Working plans call for site work for a water / sewer project.

Period of Performance: 180 days from notice to proceed The magnitude of this construction project in terms of estimated cost is between $500,000 and $1,000,000 The award will be a firm fixed price contract. Award will be based on the lowest evaluated price of the technically acceptable proposal meeting all of the solicitation requirements from responsible offerors with acceptable past performance. There will be one award made for the solicitation. Award may be made without discussions. The following summary is for the purpose of general information and is not intended to include or describe every feature or item or to define the scope of work. It is expected that prospective contractor will attend the site visit and thoroughly familiarize themselves with the project prior to submitting a proposal. In addition, the complete solicitation, statement of work, and drawings will be available on the solicitation issuance date. The Contractor shall furnish all labor, material, supplies, equipment, tools, delivery, disposal and supervision required in performing all operations in conjunction with this project complete, in strict accordance with these specifications for Project CSC-19731 and Contract Drawing No. 7-729 subject to the terms and provisions of this contract. Mandatory Contractor Qualifications: To be Technically Acceptable, qualified offerors must provide information regarding the technical qualifications of the proposed project staff, to include the individuals who will be responsible for the following functions: Written qualification statement for the individual who will be responsible for Project Management of the project. Qualifications The Project Manager must have a minimum of 5 years' experience as a Project Manager or Superintendent on projects of similar size and complexity. Written qualifications statement for the individual who will be responsible for Quality Control in accordance with General Requirements Section 01 45 23, Paragraphs 4 and 5. Qualifications An individual with a minimum of 5 years combined experience as a superintendent, inspector, QC Manager, project manager, or construction manager on similar size and type construction contracts which included the major trades that are part of this contract. The individual must be familiar with the requirements of the EM 385-1-1 and have experience in the areas of hazard identification and safety compliance. Construction Quality Management Training In addition to the above experience and education requirements, the QC Manager shall have completed the course Construction Quality Management for Contractors and will have a current certificate. Written qualifications statement for the individual who will be responsible for safety in accordance with FAR 52.236-13 Accident Prevention (Nov 1991). Accident Prevention includes compliance with the standards issued by the Secretary of Labor at 29 CFR Part 1926 and 29 CFR Part 1910; and compliance with all pertinent provisions of the latest version of U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1, in effect on the date of the solicitation. This individual will be designated as the Site Safety and Health Officer (SSHO). Qualifications and Training The SSHO(s), as a minimum, must have completed the 30-hour OSHA Construction safety class or as an equivalent, 30 hours of formal construction safety and health training covering the subjects of the OSHA 30-hour course (see Appendix A, paragraph 4.b) applicable to the work to be performed and given by qualified instructors. > The SSHO is also required to have five (5) years of construction industry safety experience or three (3) years if he possesses a Certified Safety Professional (CSP) or safety and health degree. The written qualifications statement shall be certified and signed by a principle of the offeror. Offerors shall submit a resume of each key individual. This will be evaluated to determine if they have the appropriate technical training, certifications, and previous construction experience to meet the technical requirements of a project of this nature. If any of the submitted individuals will perform more than one of the listed functions, this is acceptable as long as the submitted documentation identifies any and all functions the given individual will be performing. Certain skilled, experienced, professional and/or technical personnel are essential for successful accomplishment of the work to be performed under this contract. These are defined as " key personnel " and are those persons whose resumes are submitted as part of the technical/business proposal for evaluation. The contractor shall use key personnel as identified in its proposal during the performance of this contract and will request contracting officer approval prior to any changes. Requests for approval of any changes shall be in writing with a detailed explanation of the circumstances necessitating the change. The request must contain a complete resume for the new key personnel and any other pertinent information, such as degrees, certifications, and work history. New key personnel must have qualifications that are equal to or higher than those being replaced. The contracting officer will evaluate the request and notify the contractor whether the requested change is acceptable to the Government. The remaining two factors to determine technical acceptability are past performance technical construction capability All individuals wishing to attend the site visit will be required to request access no later than 24 hours prior to the site visit. Those individuals wishing to attend must provide their name and corporate affiliation to the contracting officer by e-mail at Karl.Hinch@dla.mil. Individuals will need to enter the Center through the Visitor Processing Center and a valid federal or state issued photo identification will be required. All questions shall be submitted via e-mail, and must be submitted no later than 5 June 2019.
