Prequalification of bidders is required. Prior to submitting a bid, the bidder must have on file with the Utah Department of Transportation a completed and approved contractor's application for prequalification. Department processing time is 10 working days from receipt of properly executed documentation. Qualified Health Insurance is required on this project. Insurance must be submitted with executed contracts. See Standard Specification 00515 - 1.11(A). As required, a contractor's license must be obtained from the Utah Department of Commerce. A prime contractor is subject to qualified health insurance requirements if the prime contract is in the amount of $2,000,000 or greater. A subcontractor is subject to qualified health insurance requirements if the subcontract is in the amount of $1,000,000 or greater.
