Demolition and site work for a residential development in Clermont, Florida. Completed plans call for the demolition of a residential development; and for site work for a residential development.

The Contractor shall obtain all permits, licenses and certificates as may be required by Federal, State, and Local laws, ordinances, rules and regulations for the proper execution and completion of the work specified. The septic tank is located along the left front of the structure. The septic tank will not be abandoned as part of this project. The contractor is to use caution when working in this area as to not damage the septic tank. The septic tank corners will be marked with wooden stakes and survey ribbon. The contractor shall be responsible for capping the remaining PVC line leading from the structure to the septic tank. Property information can be obtained by visiting the Lake County Property Appraisers electronically. There is a NESHAP Survey for Asbestos containing materials for this property attached to this RFQ. Demolition services shall be in accordance with the applicable NESHAP Survey and shall include but not limited to: the demolishing and removal of the existing structure, footings, concrete floors, small concrete slab located on the east side of the structure, and a portion of both the asphalt and concrete apron located on the west side of the structure. The portion of the driveway apron to be removed is marked with pink paint. The well that services this structure is located on the west side of the driveway and is to remain in place. The contractor is to use caution when working in this area as to not damage the well. The existing power source and associated piping leading from the structure to the well will be damaged during the structure demolition. The contractor shall not responsible for repairing said damage. All debris shall be collected and removed from the site and properly disposed of. The entire site shall be graded, leveled and left in good condition. Once the site has been leveled, the contractor shall be required to seed and mulch all disturbed areas with a mixture of brown top millet and bahia grass set at a rate of one hundred pounds (100 lbs) per acre. All waste generated by this demolition shall be disposed of at a state permitted landfill. Lake County shall require invoices verifying debris has been taken to a state permitted landfill. All utilities connected to the structure have been terminated. The contractor shall take the necessary precautions and bear the responsibility for the safety of the methods employed in performing the work. The contractor shall comply with the regulations set forth by federal, state, and local laws, rules and regulations concerning OSHA and all applicable state labor laws, regulations and standard. All prices submitted are to be on the form below in accordance with all terms and conditions set forth in this Request for Quotation. Prices quoted should be in unit of measure shown. Any award resulting from this RFQ will be made to the responsive, responsible vendor which offers the lowest price on an aggregate basis. If award is noted to be made on an aggregate basis, any vendor response that fails to include pricing for all items may be rejected. Prices shall be quoted F.O.B. Destination - inside delivery, freight included and shall be inclusive of all costs. Current and/or anticipated applicable fuel costs should be considered and included in the price quoted. Work must be completed within 14 days after issuance of purchase order or notice to proceed. Each vendor shall include in its solicitation response package proof of insurance capabilities, including but not limited to, the following requirements: [This does not mean that the vendor must have the coverage prior to submittal, but, that the coverage must be in effect prior to a purchase order or contract being executed by the County. An original certificate of insurance, indicating that the awarded vendor has coverage in accordance with the requirements of this section, shall be furnished by the vendor to the Contracting Officer within five (5) working days of such request and must be received and accepted by the County prior to contract execution and/or before any work begins.
