Site work and paving for a civil project in Oklahoma City, Oklahoma. Completed plans call for site work for a bridge / tunnel; for paving for a bridge / tunnel; road / highway; water / sewer project; road / highway; and water / sewer project.

The installation, maintenance, and data collection procedures for the pipe riser settlement devices used to monitor subsurface settlement are described in this special provision. Settlement devices are used to monitor the rate and magnitude of settlement occurring at a point within or beneath an embankment during and subsequent to construction. The data obtained from these devices are used to determine the allowable loading rate during embankment construction and/or commencement of permanent structure construction. The pipe riser settlement device is a direct-reading unit which is exposed for the full duration of fill construction. The purpose of the Contract is to provide details for the construction and completion of the Work. The Contractor is responsible for providing all labor, materials, equipment, and incidentals required to complete the work in accordance with the Contract. No alterations based upon the increase or decrease in pay item quantities may be entered into that increase the length or extent of a project by more than 25 percent. As necessary, the Department may prepare Special Provisions describing work included in the Proposal Forms. The Department will consider Special Provisions attached to or incorporated by reference in the Proposal Forms to be part of the Contract. During the progress of the work, if subsurface or latent physical conditions are encountered at the site differing materially from those indicated in the Contract or if unknown physical conditions of an unusual nature, differing materially from those ordinarily encountered and generally recognized as inherent in the work provided for in the Contract, are encountered at the site, the party discovering such conditions shall promptly notify the other party in writing of the specific differing conditions before the site is disturbed and before the affected work is performed. Upon written notification as provided in Subsection 104.06, "Notification of Differing Site Conditions, Changes, and Extra Work," the Resident Engineer will investigate the conditions, and if it is determined that the conditions materially differ and cause an increase or decrease in the cost or time required for the performance of any work under the Contract, an adjustment, excluding anticipated profits, will be made and the Contract modified in writing accordingly. The Resident Engineer will notify the Contractor of the determination whether or not an adjustment of the Contract is warranted. No Contract adjustment which results in a benefit to the Contractor will be allowed unless the Contractor has provided the required written notice as specified in Subsection 104.06, "Notification of Differing Site Conditions, Changes, and Extra Work." No Contract adjustment will be allowed under this clause for any effects caused on unchanged work. If the Contractor has provided written notification of differing site conditions as provided in Subsection 104.06, "Notification of Differing Site Conditions, Changes, and Extra Work," and the Resident Engineer determines that differing site conditions exist, payment will be made in accordance with Subsection 109.04, "Differing Site Conditions, Changes, and Extra Work," and adjustments to Contract Time will be made as provided in Subsection 108.07, "Administration and Extension of Contract Time." The installation, maintenance, and data collection procedures for the vibrating wire piezometer devices used to monitor subsurface pore water pressure are described in this special provision. Vibrating wire piezometer devices are used to monitor the changes in pore water pressure occurring at a point within or beneath an embankment during and subsequent to construction. The data obtained from these devices and settlement plate devices are used to determine the allowable loading rate during embankment construction and/or commencement of permanentstructure construction. The vibrating wire piezometer device is a directreading unit which is exposed for the full duration of fill construction. The instrument locations shall be as shown on the plans or as directed by the ODOT Engineer. Monitoring of the instruments will be performed by the ODOT Engineer. This work includes: o Furnishing, installing and maintaining instruments and associated equipment to monitor dissipation of foundation pore pressures during construction of approach embankments. o Testing, calibrating and establishing base line readings to verify that instruments are working and properly installed following installation, and removal or abandonment of instruments at the conclusion of the project. o Designing layout and selecting locations of wiring from vibrating wire piezometers and settlement cells to the readout boxes, including locations for the vibrating wire terminal boxes for collecting the data. o Submittals providing details of the specific types, makes and models of the various instruments proposed for use on the project, including catalog cuts and manufacturers certifications, shop drawings, and methods of installation for the Engineer's approval. This section establishes procedures for determining acceptability and pay adjustments as they relate to smoothness requirements of pavements and bridge decks. The equipment and testing applicable to this section shall be provided and/or operated by the party or parties specified in Appendix A of this provision. Except as noted herein, these special provisions apply to all types of Portland cement and asphalt concrete pavements, as well as bridge decks constructed as part of this contract, or as specified on the plans. This work consists of providing electronic data for material weight tickets of all bituminous material delivered to the project. This work also consists of placing an identifying vehicle number on the driver side and the passenger or rear sides of the delivery vehicle. These provisions do not preclude or dismiss any requirement for paper tickets required by the Standard Specifications or other rules and regulations. This work consists of obtaining and providing construction traffic surveillance by the Oklahoma Highway Patrol (OHP) on highway construction projects. The use of OHP and the number of patrol cars required on the project is at the approval and/or discretion of the Engineer. This work consists of excavating material and constructing embankments, which includes hauling, stockpiling, placing, disposing, sloping, shaping, compacting, and finishing earth material. There are three categories of earthwork -- excavation and borrow, embankment, and lump sum earthwork. The training of minorities, women, and socially and economically disadvantaged individuals toward journeymen status is a primary objective of this OJT Program. Accordingly, the Contractor shall make every effort to enroll eligible Trainees to the extent that such persons are available within a reasonable area of recruitment. The Contractor is responsible for demonstrating the steps taken in pursuance thereof prior to a determination of compliance with this Special Provision. This training commitment is not intended, and shall not be used, to discriminate against any applicant for training, whether the applicant is a member of a minority group or not. The prospective Trainee must express interest in entering the OJT Program, as well as exhibit sufficient commitment to completing the training. The requirements and procedures for the issuance and distribution of material certifications are designated in the FAST Guide. Materials not addressed specifically by the FAST Guide will be handled in accordance with the Contract requirements. This work consists of constructing, maintaining, and removing the following items at locations shown on the Storm Water Pollution Prevention Plan (SWPPP): o Temporary slope drains to avoid sheet flow on slopes; o Temporary bale barriers to trap sediment at the toes of slopes, or across ditches and defined waterways; o Temporary silt fences to prevent sediment from moving off the right-of-way; o Temporary sediment filters to trap sediment and debris; o Temporary sediment basins to collect sediment-laden runoff from disturbed areas; o Temporary silt dikes to trap sediment at the toes of slopes, or across ditches and defined waterways; o Temporary rock filter dams to dissipate the energy of flowing water and collect sediment near the toes of slopes, at upstream and downstream drainage structures, and in roadway ditches or defined waterways.
