The Following Information Must Be Included Before the Plans & Specs Will Be Released: Company Name Contact Name Contact Email Address Mailing Address Phone/fax #'s Each Proposal Shall Be Signed by the Full Name and Business Address of Each Person or Company Interested in the Same; Shall Be Accompanied by a Security Bond by a Bonding Company Authorized to Do Business in the State of Ohio, or by a Certified Check on Some Solvent Bank in the Amount of Ten Percent (10%) of the Amount of the Submitted Proposal, the Surety Bond or Certified Check Being Payable to the City of Moraine, Ohio, as a Guarantee That if the Proposal Is Accepted, a Contract Will Be Entered Into and Its Performance Properly Secured by a Satisfactory Bond in the Amount of One Hundred Percent (100%) of the Contract Price. The Proposals Must Be Made on a Hard Copy of the Forms Provided by the City of Moraine in the Contract Documents Section, or an Unaltered Copy Thereof, With Either a Lump Sum Bid Amount or a Total Bid Amount With a Price Quoted for Each Item of Work. The Price of Labor and Materials Is to Be Separately Stated.
