Submit one fully executed copy of AIA Document A305 Contractor Qualification Statement prior to submitting this bid form to Keith Demchinski (keith.demchinski@kimley-horn.com) The Owner Representative for this project is KimleyHorn & Associates. All questions concerning this project or those concerning bidding requirements should be directed to Keith Demchinski at 630-487-3450. Questions must be received in writing, or via email (keith.demchinski@kimleyhorn.com ), until 12:00 PM, Tuesday, September 24, 2019. The competency, experience and responsibility of the bidders will be considered in making awards. Bid security in the form of a Bid Bond, certified or Bank Draft in the amount equal to not less than 10% of the bid and made payable to the Glen Ellyn Park District. All Contracts are subject to the Illinois Prevailing Wage Act (820 ILCS 130/1-12).
