Subject to conditions prescribed by the University of California, Berkeley, sealed bids for a sealed bids for a lump-sum contract are invited for the following work: Boalt Lot Repaving PROJECT NO. 16002A UNIVERSITY OF CALIFORNIA BERKELEY, CA; ALAMEDA COUNTY Repave and restripe existing parking lot including minor grade modifications as shown on project drawings and specs. All onsite work must begin on or after December 23, 2019 and be substantially complete by January 13, 2020. To allow the University to evaluate bidders, each bidder must submit with its bid, a Contractor's Special Statement of Experience. This Contractor's Special Statement of Experience must be provided on the Contractor's letterhead. If your firm provides inaccurate or incomplete information your firm will be found non-responsive. If your firm receives negative project references they may be found non-responsive. To be eligible for consideration of award, the Contractor's Special Statement of Experience must include: 1. Documentation that the bidder has successfully completed the following: 1. A minimum of three (3) and a maximum of five (5) projects completed in the past five (5) years of similar scope and demonstrate the bidder's ability to successfully complete the project with respect to project size, use, complexity, construction value of $345,000 or greater, etc. DO NOT include projects currently under construction. 2. All submitted projects must be within the immediate nine Bay Area counties. 3. Include the following for each project: (a) project name, (b) owner's name, (c) contract value at award, (d) total Change Order value, (e) completion date, (f) Owner's Representative address and current phone number and (g) brief scope. 2. All projects on all UC Campuses within the last 5 years. 3. Statement must include the following declaration: I, authorized agent of , (Name of Firm) hereby certify, under penalty of perjury under the laws of the State of California that the foregoing is true and correct. In the last ten (10) years that the organization, any affiliate, parent or subsidiary company, or the principals, officers or RMO's of the organization, joint venture, any affiliate, parent or subsidiary company has not been found by a judge, arbitrator, jury, or a nolo contendere plea to have submitted a false or fraudulent claim. The successful Bidder will be required to have the following California current and active contractor's license at the time of submission of the Bid: "A" - General Engineering Contractor Every effort will be made to ensure that all persons have equal access to contracts and other business opportunities with the University within the limits imposed by law or University policy. Each Bidder may be required to show evidence of its equal employment opportunity policy. The successful Bidder and its subcontractors will be required to follow the nondiscrimination requirements set forth in the Bidding Documents and to pay prevailing wage at the location of the work. The work described in the contract is a public work subject to section 1771 of the California Labor Code. No contractor or subcontractor, regardless of tier, may be listed on a Bid for, or engage in the performance of, any portion of this project, unless registered with the Department of Industrial Relations pursuant to Labor Code section 1725.5 and 1771.1. This project is subject to compliance monitoring and enforcement by the Department of Industrial Relations. The successful Bidder shall pay all persons providing construction services and/or any labor on site, including any University location, no less than the UC Fair Wage (defined as $13 per hour as of 10/1/15, $14 per hour as of 10/1/16, and $15 per hour as of 10/1/17) and shall comply with all applicable federal, state and local working condition requirements.
