FEMA Project Worksheet Plantwide Work Orders. Work to be performed under Contract No. B080 includes, but is not limited to, the furnishing of all safety, supervision, labor, materials, supplies, equipment and all other facilities required by the Contract Documents for Plantwide improvements at the PVSC Wastewater Treatment Plant located at 600 Wilson Avenue, Newark, New Jersey 07105. All work will be performed as Task Orders, which will be identified by PVSC and the Program Manager (Engineer).The specific scope of work for each Task Order will be developed by the selected Contractor and the Engineer. The not-to-exceed fee will be assigned based on negotiated hours an d equipment, using the bid labor and equipment rates set in this Contract. The Task Order will be signed and issued by PVSC. Contractor will take responsibility for the performance of the installation of each task order. Each bid must be accompanied by a certified check, or by a bid bond in an amount not less than 10% of the total bid, not to exceed $20,000.00 enclosed in a sealed envelope, with the bid. Certified checks shall be drawn upon a National Bank or a Trust Company doing business in the State of New Jersey, and shall be payable to the order of the Passaic Valley Sewerage Commission. Bid bonds shall be prepared on the form of Bid Bond attached hereto in Section 00301, and shall be duly executed by the bidder as principal and having as surety thereon, sureties or a surety company shall be licensed to do business in the State of New Jersey, and listed in the current Federal Register, Department of the Treasury Circular 570, "Surety Companies Acceptable on Federal Bonds", and shall be satisfactory to the Passaic Valley Sewerage Commission. All bidders on this Contract are required to complete a "Statement of Ownership" found in Section 00303. Bid securities will be returned to all except the three (3) apparent lowest responsible bidders within ten (10) days after the formal opening of bids, (Sundays and Holidays excepted) and the remaining bid securities will be returned to the unsuccessful bidders within three (3) days (Sundays and Holidays excepted) after the awarding and signing of the contract and the approval of the Contractor's performance bond, or if no Contract has been so executed, within 60 days after the date of the opening of bids, upon demand of the bidder at any time thereafter, as long as he has not been notified of the acceptance of his bid. The Passaic Valley Sewerage Commission reserves the right to reject all bids pursuant to N.J.S.A. 40A:11-13.2, to waive minor informalities and to award the contract to the lowest responsible bidder in accordance with applicable law. Questions and request for proposals shall be directed electronically or in writing, to the Passaic Valley Sewerage Commission Purchasing Department for official response. The final day to submit questions shall be no later than 10:00 am EST seven (7) days prior to bid date, Saturdays, Sundays and Holidays excepted. If the contract is to be awarded, it will be awarded to the lowest responsive and responsible bidder. Cost of the Complete Bidding documents is one hundred dollars ($100.00) per set (non-refundable) for each set of Contract Documents ordered, payable only by business check to Passaic Valley Sewerage Commission. Bidding documents will be available for examination at the office of the Purchasing Department at Passaic Valley Sewerage Commission. The Purchasing Department telephone number is (973) 466-2723. If for some reason the Contract is not awarded, refunds of the Bidding Documents fee will be immediately returned to Bidders when Bidding Documents are returned in reasonable condition within 90 days of notice that the Contract has not been awarded.
