Liquidated Damages - N/A Request for Pre-bid substitutions for products or materials. The Request for Pre-bid substitution for product or materials form must be submitted by this date December 3, 2019to be considered for review. The purpose of this Request for Bid (RFB) is to solicit responses from responsible, responsive biddersto provide the goods and/or services identified and described belowass pecifically described in Section 2of this solicitation. Performance and Payment Bond(If applicable)If the contracted amount is $25,000 or more,the successful Bidder must furnish a performance bond covering the faithful performance of 100% of the Contract and the payment of all obligations arising thereunder. One copy of the bond mustbe submitted on Iowa Department of Transportation Form 131070.All items must be properly filled in, including Bidder's signature.A Resident Commission Agent or attorney-in-fact must file a copy of the power of attorney. The Iowa DOT must receive responses either electronically or by standard mailon or beforethe deadlineon the Solicitation Response page. Any responses received after this deadline may be rejected. Biddersmust furnish all information necessary to be considered for award. Responsesthat fail to meet the mandatory requirements of the RFBwill be disqualified. Verbal information provided by bidders mustnot be considered part of the bidders'response. It is understood that the Iowa DOT considers this a best estimate onlyof requirements and makes no firm quantity commitment Successful Bidder mustbe required to provide all materials, labor, and equipment necessary for thereplacement of the Northeast Sanitary Sewer Pipe. Contractor to remove and replace approximately 100' of 4" cast iron sanitary sewerline. Replacement shall be replaced with cast iron.Contractor to isolate, drain, remove, and dispose of approximate 100' ft of 4" sanitary sewer.Contractor to install a double cleanout per code next to building to provide future maintenance.Contractor to install a single cleanout at the sidewalk.Cleanout to be clearly marked with 60" dual sided utility post or equal. Contractor to provide and install new 4" piping and fittings to properly connect to existing 4"pipe coming into the building. Install a tracer wire for future locates before backfilling.Contractor is responsible for offsite disposal of debris.LicensedPlumber is required. All work to be compliant with local plumbing codes.All work to be compliant with OSHA Standards.Contractor to notify DOT authorized personal of any damaged or defective parts uncovered immediately. A signed Change Order is required for any change to the scope of work, prior to the work being completed.Contractor to properly backfill and compact using pea gravel, sand, and soil. Contractor to provide proper SDSsheets on all hazardous chemicals being used.Contractor to provide all locates.Contractor to notify DOT 24 hours prior to starting work. Normal Iowa DOT business hours 6:30am-4:00pm Mon-Fri. Contractor to provide anticipated work schedule for approval of hours worked during and after business hours including weekends. The Successful Bidderfor the project mustfurnish the Iowa DOT with a complete list of subcontractors and major material suppliers at the pre-construction meeting. The Iowa DOT mustapprove and maintain the list of subcontractors and major suppliers andissue a general approval of same after official award of the contract, subject to the specific requirements of the Plans, Specifications and the "General Conditions of the Contract, and of these supplementary Conditions,SpecialProvisions, and elsewhere with contract documents, as applicable. Deviations from the list of subcontractors and material suppliers mustbe made only with the specific approval of, or at the request of the Iowa DOT.
