Site work and paving for a road / highway in Senoia, Georgia. Conceptual plans call for site work for a road / highway; and for paving for a road / highway.

The City of Senoia requests Statements of Qualifications from consultants to provide professional civil engineering and material testing services for the subject project. Submitting firms must be pre-qualified by GDOT. Personnel assigned to the project must be PDP certified or GDOT certified for testing, as applicable. Basic Services include: Assigned engineer to perform monthly inspections to verify material quantities installed, approve workmanship, and approve contractor pay applications. Eight (8) monthly site visits/inspections are anticipated. Assigned engineer to attend a monthly status meeting throughout the project cycle to provide guidance on anticipated work activities. Eight (8) monthly status meetings are anticipated, and should be held concurrent with monthly inspection dates (above) in order to reduce trips and engineering costs. Assigned engineer to attend the preconstruction conference. Assigned engineer to address contractor requests (RFIs), material approvals, substitutions, clarifications, etc. via phone, e-mail or letter as needed. Assigned engineer to assemble and submit the quarterly and final MC Checklist in compliance with GDOT standards (4 MC checklists total anticipated (3 quarterly, 1 final)). This includes assembling all material receipts, product cut sheets, delivery tickets, test results, certifications, etc. as needed to complete the MC Checklist. Qualifications should be limited to 15 pages, excluding resumes of assigned personnel and firm experience, and should contain at least the following items: a. Experience of any sub-contractors/consultants. Include an annotated list of related work completed or in progress. b. Summary of the qualifications of all key personnel assigned to this project. Please note if these personnel are prequalified with GDOT (i.e. - included on firm's most recent prequalification submittal to GDOT). c. Annotated list of references (with contact names and telephone numbers). d. Certificate of Professional Liability Insurance coverage indicating limits of coverage. The insurance must be written by a licensed Georgia agent or a company licensed to write insurance in the State of Georgia. e. GDOT certifications. 4. Qualifications must be typed or printed in ink. No changes or corrections will be allowed after qualification packages are opened. 5. Proposers are expected to submit their qualifications to perform the services identified. Each proposer shall examine these documents carefully; and, no later than seven calendar days prior to the date for receipt of qualifications, shall make a WRITTEN request to Harold Simmons, City Administrator for interpretations or corrections of any ambiguity, inconsistency or error which may be discovered. All interpretations or corrections will be issued as addenda and will be sent to all proposers. No addenda will be issued during a period beginning 48 hours before opening of qualification packages. 6. The qualification package must contain a manual signature of an authorized representative of the firm on the cover sheet for proposal. 7. Qualifications received prior to the time of opening will be secured unopened. Qualifications received after the scheduled receipt time will not be accepted. 8. REJECTION OF QUALIFICATIONS: The City of Senoia reserves the right to reject any and/or all submittals, in whole or in part. The successful firm will have thirty (30) days from the date of award of the proposal to execute a contract with the City. 10. FORM OF AGREEMENT: The selected firm shall present the contract to be reviewed by the City. 11. SELECTION PROCESS: Qualifications will be evaluated by City Staff and/or a selection committee. Firms (or teams of firms) with the top submittals may be asked to make formal presentations to the selection committee. All submittals will be acknowledged in writing. Firms selected for personal appearances will be notified regarding the time and date of their interviews. A contract may be awarded based on these evaluations/interviews. However, any or all qualification packages may be rejected if it is deemed to be in the best interests of the City of Senoia. 13. EVALUATION CRITERIA: The following criteria, as a minimum, will be used to evaluate qualifications: Nature and quality of previously completed and related work; Qualifications of personnel - with emphasis on lead/contact person; Commitment to complete work on a timely basis; and, Intangible factors. ACCEPTANCE OF QUALIFICATION PACKAGES: The City reserves and holds the following rights and options: To reject any and all firms submittals; To re-advertise if deemed necessary; To interview candidates prior to making a selection; To issue subsequent Requests for Qualifications; To not negotiate or contract for the services; To approve, disapprove, or cancel all work to be undertaken; and, To designate another public body, agency, group, or authority to act in its' behalf for contract negotiations. No reimbursement will be made by the City of Senoia for any costs incurred prior to a formal notice to proceed should a contract award result from this solicitation. 15. COMPLIANCE WITH GEORGIA SECURITY AND IMMIGRATION ACT: All contractors and subcontractors shall complete and submit City of Senoia E-verify Affidavit with qualifications. Assigned engineer to assemble material tests (performed by engineer's material testing sub), and forward to the city for their project file. Assigned engineer to assemble and review sedimentation tests (performed by contractor), and forward to the city for their project file. Assigned engineer will also perform or assist with various other activities necessary to implement the project, as follows: Change order/time extension review (if needed), coordinate and attend GDOT final inspection, prepare and distribute punch list, review and forward final as-builts/redlines to GDOT and City, verify/inspect erosion control prior to ground disturbing activities, assist with acquisition of NOI, GDOT coordination Material Testing: The project is anticipated to require GDOT-certified RTT testing and concrete testing. Material tests will be coordinated by the assigned Engineer as requested by the prime contractor. A specific number of tests have not been determined. Therefore, the City has set aside a lump sum of $15,000.00 for the anticipated testing services. Increases in testing costs will be considered by the City on an as-needed basis as work progresses. The selected Engineer/Testing firm shall submit GDOT form OMAT-LAP-1v8 for GDOT approval prior to performing work activities. Notes: 1) No utility relocations are anticipated for project implementation. 2) Since the trail is located on new location, minimum traffic control is anticipated. 3) A City representative shall review contractor certified payrolls, review contractor subcontracts, monitor environmental compliance, monitor contractor DBE compliance, and perform CUF reviews. Project Description: The project would provide a 10-foot wide multi-user trail on new location west of Keg Creek in Senoia. The project begins at Seavy Street, extends north approximately 1 mile on new location, and ends at Autumn Creek Drive. The project would require approximately 1,800 linear feet of boardwalk to cross wetlands and/or floodplains associated with an unnamed tributary of Keg Creek. Payments under the Contract shall be made upon submittal of a monthly invoice after performance of services which each invoice represents. A standard GDOT progress report shall be submitted with each invoice. Project construction is anticipated to begin in August, 2020 and continue for approximately 9 months. An additional two month period to perform project close-out and assemble/submit required documentation to GDOT is anticipated after work completion. The project is being funded by a combination of City and federal funds through reimbursements from the Georgia Department of Transportation (GDOT). The City of Senoia will negotiate a lump sum fee with the most qualified consulting firm. All necessary services and expenses are to be included in the lump sum fee. The method of contractor selection has not been determined at this time.
