Proposed improvements include installation of a 10-foot wide shared use path along the east side of SR-7 from Franklin Ave. to Labelle Ave., and the north side of Franklin Ave. from N. Fourth St. to SR-7 for a total length of 1,100 feet. Improvements include retaining walls between the shared use path and railroad, railing, crosswalks, ADA - compliant curb ramps, and addition of pedestrian pushbuttons and signal heads to the SR-7/Franklin Ave. traffic signal. Bids shall be on a unit price basis and shall be in writing on proposal blanks provided in the contract documents. Each bidder is required to furnish with its proposal, a Bid Guaranty and Contract Bond in accordance with Section 153.54 of the Ohio Revised Code. Bid security furnished in Bond form, shall be issued by a Surety Company or Corporation licensed in the State of Ohio to provide said surety. The successful bidder will be required to furnish and pay for a satisfactory performance and payment bond or bonds. The Contractor shall be ODOT pre-qualified for types of work he is performing with his own forces (not subcontracting) for items that total a minimum of 30% of the awarded contract amount. To ensure the Contractor retains 30% of the awarded contract amount, a completed ODOT Prequalification Data Sheet is required to be submitted as part of the bid. In addition, the Contractor will be required to submit a job cost report with all pay requests to ensure the 30% requirement is attained throughout the project. Each proposal must contain the full name of the party or parties submitting the proposal and all persons interested therein. Each bidder must submit evidence of its experiences on projects of similar size and complexity. The owner intends and requires that this project be completed no later than June 30, 2021. Contractor compliance with the equal employment opportunity requirements of Ohio Administrative Code Chapter 123, the Governor's Executive Order of 1972, and Governor's Executive Order 84-9 shall be required. Attention of bidders is called to all of the requirements contained in the bid packet, particularly to the Federal-Aid wage rate requirements, various insurance requirements, various equal opportunity provisions, Buy America provisions and DBE requirements.
