Additionally, failure to submit all of the information on Standard Form CPDP (dated 06/07/06) shall be considered non-responsive and may result in the Qualification Statement being rejected. Responses to this RFQ will be collected, compiled, and delivered to Board members on or before December 8, 2020 so that they may review them and prepare for the selection meeting.
