The Agency plans to contract with a single entity for both design and construction ( Design- Build Entity or DBE ) of an outdoor art monument commemorating the devastation and impact of HIV/AIDS worldwide. The scope of work for the Project includes all disciplines necessary to complete the remaining design and perform the construction of the Monument, including, without limitation, all on-site and off-site work. The RFQ process is complete, and this is the second part of the process. The selected DBE will be required to take affirmative action to ensure against discrimination in employment practices on the basis of race, color, national origin, ancestry, gender, religion, sexual orientation, or handicap. It is agreed that all Contractors and subcontractors of whatever tier, if awarded contracts for work covered by this Agreement, shall be required to accept, and be bound to the terms and conditions of a Project Labor Agreement, and shall evidence their acceptance by the execution of the Letter of Assent prior to the start of any work. Any questions addressing the interpretation or clarification of the Statement of qualifications Forms, or the Statement of qualifications Documents must be submitted no later than Monday, 5:00 p.m. (PST) on August 15, 2022, through the City's electronic submission process. Following this deadline, no questions regarding RFP packet will be accepted.
