Timely reporting of lab results to the Engineer is required. This scope of work is not all-inclusive; the final scope of work will be agreed upon and in the executed contract arising from this RFP. Replacement of the existing Floradale Avenue Bridge No. 51C-006 with a new 4-span cast-in-place post-tensioned box girder bridge on 10-foot diameter CIDH piles at each pier and seat abutments on 4-foot diameter CIDH piles. The replacement Floradale Ave. Bridge No. 51C-0370 and approach road will be constructed parallel to the existing bridge on a new elevated embankment. Additional items of work include HMA roadway, dry wells, rock slope protection, fence, aesthetic concrete bridge approach railing, crash cushions, erosion control, and other appurtenances. The County of Santa Barbara Public Works Department (Public Works) is pleased to present this request for proposals (RFP) to local Caltrans/AASHTO certified materials testing vendors. Those (equipment and personnel) responding to this RFP must be currently certified in all test methods presented in this RFP. The quality assurance testing required is for the concrete deep foundations, rebar splices, concrete columns, abutments and superstructure. In addition, there is earthwork and roadway work that requires sampling and testing. This RFP is for Material testing to ensure quality assurance on this Federal Highway Bridge Replacement Program construction project. Only performing the material tests and providing material test data is required. The work requested via this RFP does not provide an evaluation or a discipline report and is therefore a non-A&E service. The project site is located near the City of Lompoc as shown on Figure 1-Vicinity Map of the RFP. The method of Contractor Selection has not been determined at this time.
