The Village of Hazel Green, Wisconsin will receive separate sealed unit price BIDs for the following items as they relate to furnishing of all labor, materials, equipment, testing and miscellaneous for the 200,000 Gallon Tower Miscellaneous Repair and Maintenance Painting contract. The existing Elevated Water Storage Tank (Tower) is a 200,000-gallon, single pedestal, welded steel structure, with a 128' overflow height, constructed in the late 1970's by Universal Tank. The existing tower was most recently repainted in October 2013. Since then, various telecommunications and internet service equipment and appurtenances have been installed within and upon the structure. These installations have resulted in damage to painted surfaces of the structure, and in some cases substandard work now requiring maintenance and corrective work. Other more typical maintenance work to components of the original structure is also required. Contract: Miscellaneous Repair and maintenance Painting - Shall include, but not be limited to - Miscellaneous repair; field surface preparation with collection and disposal of all waste materials; field prime coat materials and application, field intermediate coat materials and application, field finish coat materials and application for the Exterior Dry, Interior Dry, Interior Wet, Disinfection, Welding with collect and dispose of all waste materials; provide necessary protection of adjacent properties and structures. Contractor shall provide all labor, materials, and equipment necessary, insurance and bonds to perform this work. A satisfactory bid bond executed by the Bidder and a licensed surety company or a certified check payable to the Owner in an amount not less than 5% of the maximum BID, shall accompany each bid as a guarantee that, if the BID is accepted, the Bidder will execute and file the proper contract and bonds within ten days after the award of the contract. The letting of the work described herein is subjected to the provisions of Section 66.0901 and 66.0903 of the Wisconsin Statutes. The minimum wage rates to be paid for the contract shall be in accordance with Section 66.0903 of the Wisconsin Statutes. No State or Federal Wage Rates Determinations have been established for this project. Contractor shall carry Public Liability Insurance and Workmen's Compensation Insurance that is satisfactory to the Owner. The successful Bidder for this Contract will be required to furnish a corporate performance bond and payment bond in an amount equal to one hundred percent (100%) of the contract price, said bonds are to be issued by a responsible surety approved by the Owner. Owner reserves the right to waive formalities in bidding and to reject any and all bids.
