Regulatory Requirements: Bidder certifies that ail laws of the State of Illinois and ordinances of the City of Galesburg in effect at the date of bidding shall be observed by bidder. Evidence of any violation during the term of the agreement shall be consideredsufficient reason to discontinue purchases by the City from that supplier. B. Project Identification: Galesburq Site and Building Deconstruction, Project No: 2013302.03 Project Location: 264 W. Main Street, Galesburg, II 61401 C. Owner: Galesburg Public Library Board of Trustees 1. Owner's Representative: Martin Reichel, cell 309-351-2312, mreichei2@comcast.net D. Architect: FEH DESIGN, 951 Main Street, Dubuque, Iowa 52001, (563) 583-4900 Project Description: Project consists of the following: 1. Deconstruction of buildings and site elements on one full City block in Galesburg, IL in preparation for a future library building. 1. The deconstruction and demolition of the entire (5) buildings, footings and foundations, and structures in their entirety. 2. The intent of the demolition is to recycle the majority of building components and materials and divert from the landfills. 3. Asbestos abatement to the extent indicated in the existing hazardous material reports. 4. The termination and removal of all above ground and below grade utilities on the property as indicated on the drawings. 5. The removal of all pavement, pads and stoops, fencing, and associated trees and vegetation including root systems, allowing for grading and seeding of all areas disturbed. 6. Removal of the existing billboard, foundation, and associated structures located off W. Main Street. The City Block is bordered by W. Main Street, S. West Street, W. Simmons Street, and S. Academy Street. F. Construction Contract: Bids will be received for the following Work: 1. General Contract (all trades) including demolition and asbestos abatement. Award of the Contract shall be to the lowest responsive, responsible bidder. Neither the Library Board of Trustees nor its agents will assume liability for the inability of the bidder to submit a bid in a timely manner. Bids received after the deadline will be rejected. Bidders bear full and complete responsibility for the timely submission of such bid. Time of receipt shall be the time recorded and determined by the Library. ALL BIDDERS ARE REQUIRED TO OBTAIN AND COMPLY WITH THE REQUIREMENTS IN THE INVITATION TO BID DOCUMENTS. BID SECURITY A. Bid security shall be submitted with each bid in the amount of 10 percent of the bid amount. No bids may be withdrawn for a period of 60 days after opening of bids. Owner reserves the right to reject any and all bids and to waive informalities and irregularities. PROJECT INFORMATION A. Project Scope: The Work of the Project is defined by the Contract Documents and consists of the following: 1. The deconstruction and demolition of the entire (5) buildings, footings and foundations, and structures in their entirety. 2. The intent of the demolition is to recycle the majority of building components and materials and divert from the landfills. 3. Asbestos abatement to the extent indicated in the existing hazardous material reports. 4. The termination and removal of all above ground and below grade utilities on the property as indicated on the drawings. 5. The removal of all pavement, pads and stoops, fencing, and associated trees and vegetation including root systems, allowing for grading and seeding of all areas disturbed. 6. Removal of the existing billboard, foundation, and associated structures located off W. Main Street. A. Successful bidder shall begin the Work on receipt of the Notice to Proceed and shall complete the Work within the Contract Time for substantial completion on 30 April 2021. BIDDER'S QUALIFICATIONS A. Bidders must be Registered by the City of Galesburg. Interested bidders must contact the City of Galesburg Clerk's Office to register. B. Bidders must be properly licensed under the laws governing their respective trades and be able to obtain insurance and bonds required for the Work. A Performance Bond, separate Labor and Material Payment Bond, and insurance in a form acceptable to Owner will be required of the successful Bidder. C. Bidders may qualify to submit bids or proposals, only if they are a legal entity authorized to do business in Illinois prior to submitting the bid, offer, or proposal. This applies to both in-state and out-of-state firms. D. A Vendor / Bidder must be: 1. A legal Entity 2. Registered to conduct business in Illinois 3. In good standing with the Illinois Secretary of State STATE SALES TAX: A. This project is tax exempt. Do Not include State Sales Tax in any calculation of Bid totals. Contractor will be provided with sales tax exemption number for this project. Nonresident Contractors shall comply with Illinois Statues regarding State Tax. B. No charge will be allowed for taxes from which the City of Galesburg and the Galesburg Public Library is exempt: the Illinois Retailer's Occupation Tax, the Service Occupation Tax, theService Use Tax, the Use Tax, Federal Excise and Transportation Tax. BASIS OF BIDS A. The successful Bidder will be required to furnish a Performance Bond and Labor and Material Payment Bond in an amount equal to one hundred percent (100%) of the Contract Sum, issued by a responsible Surety approved by the Owner and shall guarantee the faithful performance of the Contract and terms and conditions therein contained and the maintenance of said improvements pursuant to the provisions of the Contract Document. Bid Security shall be made payable to Galesburg Public Library Board of Trustees, Galesburg, IL. B. Bid Security of two lowest Bidders will be retained until a contract has been awarded and executed, but no longer than 60 days. No Bidder may withdraw his bid within 60 days after opening of bids. C. The Galesburg Public Library Board of Trustees, reserves the right to reject any or all bids, readvertise for new bids, and to waive informalities that may be in the best interest of the Library. D. The bidder shall insert the prices on the Bid Form. The prices inserted shall be net and shall be the full, delivered cost, including all factors whatsoever. No permits shall be waived. Thecontractor shall be responsible to pay for all required permits to accomplish the demolition and clearance work. E. Monthly estimates will be paid to the Contractor as the work progresses in amounts equal to ninety-five percent (95%) of the Contract value of the work completed during the preceding calendar month, including the actual cost (exclusive of overhead or profit to the Contractor) of materials and equipment of a permanent nature to be incorporated in the work and delivered to and stored at the job site. Such monthly payments shall in no way be construed as an act of acceptance for any part of the work, partially or totally completed. Final payment of the five percent (5%) due each Contractor will be made upon final acceptance of the work under the respective Contract by the Owner, and after receipt of satisfactory evidence that all claims pertaining to such Contract have been paid in full as provided in the Contract Document for said work. F. By virtue of statutory authority, a preference will be given to products and provisions grown and coal produced within the State of Illinois and to Illinois Domestic Labor. G. Each bidder shall affirm that no official or employee of the City of Galesburg is directly or indirectly interested in this bid for any reason of personal gain. H. Contractors and any sub-contractors will be required to comply with all applicable provisions of the Illinois Prevailing Wage Act, as amended to date, including those affecting labor standards and prevailing wage rates and those prohibiting discrimination on the grounds of race, color, national origin and sex. I. The successful bidder is prohibited from assigning, transferring, conveying, subletting, or otherwise disposing of the contract to be signed or its rights, title or interest therein or its power to execute such Agreement to any other person, company or corporation without the previous consent and approval, in writing, by the Galesburg Public Library. J. Although the City requires copies of records such as weigh tickets (dump tickets) and certified payrolls, each vendor is ultimately responsible for retaining copies of these records for their files in the event of an audit. Standard practice is to retain these records for a minimum of seven years. Bidders' Questions: Architect will provide responses at Prebid conference to bidders' questions received up to two business days prior to conference. B. Upon completion of the Pre-bid meeting, a walking tour of the deconstruction site and each building will take place. The required Proof of site visit forms will be signed at the end of the tour. C. Each Bidder shall visit the site to familiarize themselves with conditions under which they will operate. All interested parties in attendance at the pre-bid meeting will be required to have the Proof of Site Visit form signed. There are no provisions for any additional dates for site visits. D. 10 percent Bid Security is required E. 100 percent Performance and Payment Bond is required F. The bidding and letting of Work described herein is subject to applicable provisions of the State of Illinois Statues. G. Wage rates shall not be less than prevailing wage rates as determined by the State of Illinois Department of Labor.
