As of 03/28/2022 This project will rebid. The complete carpet and cove base replacement at the DOCO Health Department The complete carpet and cove base replacement at the DOCO Health Department located at 1710 South Slappey Blvd per the attached Scope of Work. Dougherty County Health Department, approximately 62,000 ft2. 1. General Clinic (NE Wing) 2. Reproductive Health (NW Wing) 3. WIC Area/Child Health (NE Wing) 4. New Beginnings (NE Wing) 5. Administration (SW Wing) 6. Main Hallways (4 total, only carpeted areas, tile is to remain) 7. Pharmacy (SE Wing) 8. Education Center (SW Wing) 9. District Women's Health (SE Wing) 10. Dental (SE Wing) 5% Bid Bond is required of all bidders. Bid bond must be present for bid to be read or considered. 100% Performance and Payment bonds will be required of the successful bidder. The deadline for questions is 5 p.m. on February 15, 2022.Questions received after this deadline may not be answered. Replies of substance will be in the form of written addenda made available to all potential bidders. Completion time for the project is one hundred and twenty (120) calendar days following the issuance of the Notice to Proceed. Performance will be monitored and documented by the Project Manager. Not completing contract within the time specified may hamper the contractor ability to secure future contracts with the City. Should the Contractor, or, in case of default, the Surety fail to complete the work within the time stipulated in the contract or within such extra time that may be allowed, charges shall be billed to the contractor or assessed against any money due or that may become due the Contractor at a rate of Five Hundred dollars ($500) for each date that expires after the allowed contract time for the completion and readiness of final payment until the Work is complete and ready for payment. 1. Move furniture as needed to complete the project. Details will be discussed during the pre-bid meeting. 2. Remove and dispose of existing carpet and any existing cove base. a. If walls are damaged, Contractor shall repair the walls before the end of the project. 3. Prepare substrate for application of new carpet squares or luxury vinyl plank and cove base. 4. Install carpeting or luxury vinyl plank and cove base in the specified areas. The successful bidder shall provide samples of selected manufacture carpet. Color and pattern to be selected by Dougherty County Facilities Management and Dougherty County Health Department staff. The successful bidder shall provide samples of the selected manufactures LVP and commercial carpet line. Acceptable manufactures for both commercial carpet squares and LVP- Mohawk, or Shaw. Color and pattern to be selected by Dougherty County Facilities Management and Dougherty County Health Department staff. 5. Carpet is to be commercial carpet squares only. a. Carpet squares will be commercial squares, 24x24, carpet shall be designed with a shorter pile and a tight looping style with a minimum 15-year warranty. Carpet squares are to be at minimum 0.25 inches thick. Carpet squares will be installed in a rotating pattern
