Providing all materials, equipment and labor for the Kendrick Lift Station Phaseout, to include demolition of lift station, installation of 8" gravity sanitary sewer, miscellaneous asphalt pavement, sod, structural fill. Work is to be completed within 120 calendar days of issuance of Notice to Proceed. It is the responsibility of the Bidder to ensure examination of the bid documents and site and to submit any questions / requests for clarification as directed within the Bid Documents. Each Bidder must deposit with his Bid, a Bid Bond, or a Certified or Cashier's Check to the order of the Forsyth County Board of Commissioners in an amount not less than five percent (5%) of the amount bid and a Consent of Surety form from a surety company licensed to conduct business in the State of Georgia, engaging to furnish, upon award of the Contract, a Performance Bond and a Payment Bond, each in a sum not less than one hundred percent (100%) of the total price bid for the completed work and the appropriate insurance certificate. Award of bid will be made at a later date. The Board of Commissioners reserves the right to reject any or all bids. Install service connections and reconnect existing sewer service. Pressure and leak test the force main and/or gravity sewer and manholes until testing requirements of the specifications are met Backfill, compact, and repave all pipe trench cuts within paved areas; and backfill, compact, and landscape all pipe trench cuts within unpaved areas, in compliance with the Contract Documents. Restore all disturbed areas including driveways, parking areas, curbs, curb and gutter, sidewalks, yards, ornamental plantings, etc. Demolition of existing lift station components cannot occur until new sanitary sewer is installed and accepted. Demolition Work to be performed under this Contract is as described in the Notice of Bid Advertisement and shall consist of furnishing all labor, materials, tools, equipment, and incidentals and performing all work required to complete demolition of existing facilities in a workmanlike manner including the following work: 1. Remove existing trees as shown on the plan to facilitate the Work. 2. Remove existing appurtenances as outlined on the Plans; salvage and return to FCDWS, or as directed by FCDWS. 3. Pump sewage from wet well and flush the force main as required, or directed by FCDWS
