Financial Project No.: 446851-1-72-01 District Seven FCO Mill and Resurface Project Project Location(S): Florida Department of Transportation, Tampa Operations Center, 2822 Leslie Road, Tampa, FL 33619 The Florida Department of Transportation (FDOT) seeks a qualified, experienced, and licensed Contractor to perform milling, resurfacing, and striping on parking areas and roadways specified in Exhibit "A." All work shall be new and meet FDOT Standard Specification January 2021 edition, manufacturer recommendations, and FDOT flexible pavement design manual specifications. Project includes one Add-Alternate. Owner representatives will be present to discuss plans, specifications, and conditions of the project. Tavenner at (813) 975-6092 carlos.tavenner@dot.state.fl.us Vendor is required to be registered and licensed in the State of Florida for the work being performed and shall have been actively engaged in this type of work for a minimum of Five (5) years. Contractor shall have completed at least 3 (three) projects of similar size and scope within the past 5 (five) years. Each bidder shall submit a current license issued by the State of Florida and, if a Florida Corporation, a copy of the Corporate Charter as prequalification of their eligibility with the bid document to the Department of Transportation, Contracts Administration Office. After the bid opening, the lowest responsive bidder shall qualify in accordance with this contract document and after award the bidder must provide documentation of the secondary trade's licenses. Experience Questionnaire is required for this project. Copy of current CPA reviewed or audited financial statement section is required for this project. If the Bid Amount Exceeds $100,000, the Bidder Must Provide With the Bid, a Bid Guaranty of Five Percent (5%) of the Actual Total Bid in the Form of a Certified Check, Cashier's Check, Treasurer's Check, Bank Draft of Any National or State Bank, or a Surety Bid Bond Made Payable to the Department of Transportation. a Bid Guaranty Inan Amount Less Than Five Percent (5%) of the Actual Bid Will Invalidate the Bid. Bid Bonds Shall Conform to the Departments Bid/proposal Bond Form Furnished With the Proposal Package. Performance Bond and Labor and Materials Payment Bond: if the Contract Award Amount Exceeds $100,000, Both a Performance Bond and a Labor and Material Payment Bond of 100% Each of the Contract Sum Arerequired at the Time of Award. The contractor will have until 5:00 P.M. April 2, 2021 to submit questions. Questions submitted after 5:00 P.M. will NOT be guaranteed an answer, and questions asked by 5:00 P.M. will be answered by 5:00 P.M. April 7, 2021.
