For Port Huron Area School District - Garfield Elementary Parking Lot Improvements The work consists of furnishing all labor, material, and equipment required to reconstruct the existing parking lot. The work includes, but is not limited to, the following items: HMA base crushing and shaping, removal of existing pavement, excavation, grading, aggregate base for new parking areas, hot mix asphalt paving, sidewalks, pavement marking and signage, dumpster enclosure, lawn restoration, and related items of work Bids shall be on a unit price basis for items of work, as specified. Bids shall be submitted on the proposal forms provided by the Engineer Bid security in the form of a certified check or a Bid Bond, for a sum not less than five percent (5%) of the total amount bid, payable to Port Huron Area School District, shall be submitted with each bid. As required by State Law (Public Act 232 of 2004) all proposals/bids must be accompanied by a sworn and notarized statement disclosing any familial relationship that exists between the Owner or any employee of the bidder and any member of the Port Huron Area School District Board or the Superintendent of the School District. Additionally, as required by State Law (Public Act 517 of 2012), the bidder must certify that it is not an Iran-linked business as defined by PA 517. Any bid not including this sworn and notarized statement will not be accepted. The School District reserves the right to accept or reject any Bid, and to waive irregularities in Bids. The successful bidder will be required to furnish satisfactory Performance Bond, Payment Bond, Maintenance Bond and Insurance Certificates Bids may not be withdrawn for ninety (90) calendar days after receipt.
