Renovation of a medical facility in Uniondale, New York. Completed plans call for the renovation of a medical facility.
NHCC is seeking proposals from qualified organizations to provide services at A. Holly Patterson Extended Care Facility ("AHP") in connection with physical rehabilitation. Services requested include the following: Initial assessment of the rehabilitation department, including structure, function, processes, compliance, staff integration, and utilization of services. Development of a structured improvement plan based on the initial assessment. The primary objectives of the plan are the enhancement of the department function and staff knowledge (especially in the areas of reimbursement, maximizing functional independence for all residents and regulatory compliance). An audit of the Minimum Data Set ("MDS"), with regard to data capture and regulatory safeguards.
