Each bid proposal shall include all labor, material, and services necessary to complete this project in strict accordance with the plans and specifications. All perspective bidders are directed to section 102 - BIDDING REQUIREMENTS AND CONDITIONS found in the 2020 INDOT Standard Specifications. 102.01 Prequalification and Bidding. The bidder will be required to prequalify and follow the bidding procedures as set out in the rules for Prequalification of Contractors and Bidding, 105 IAC 11, now on file with the Indiana Secretary of State, copies of which are available upon request in the Contract Administration Division. In addition, Contractors and suppliers must conform to IC 5-16-13. Be sure to familiarize yourself with these changes before bidding on public works projects. Bids shall be properly and completely executed in accordance with the Instructions to Bidders and shall be submitted on the Bid 96 Form included in the Project Manual. All bids shall be itemized. The Bid Registration form (included in Project Bid Specifications) must be faxed to 260-449!-8978 to become a registered plan holder and receive addendum notifications. The bid shall also be accompanied by a bid bond in the amount of 10 % of the bid. The bid bond must be present and attached to the original bid. Bid envelope (minimum size 9" x 12") must be sealed and include company name and address, project name, bid reference number, and date and time of bid opening. Each bidder is required to include one (1) original and two (2) copies of their bid. The following documents must be submitted with the bid: Completed State Bid Form 96 (revised 2013) including the NonCollusion Affidavit Statement of Company Equal Employment Opportunity and NonDiscrimination Policy Indiana Legal Employment Declaration (E-Verify) Iran Certification Written plan for employee drug testing program W-9 (In the absence of a Federal Identification Number, a Social Security Number must be provided.) Required documents must be included with each submittal. Contractors and suppliers shall conform to IC 5-16-13. The successful bidder(s) will be required to furnish insurance covering Workmen's Compensation, Public Liability and Property Damage and any other which may be required, before the contract can be signed and issued. Bids shall not be modified, withdrawn or canceled, without the Owner's written consent, for a period of Sixty (60) calendar days commencing from the day bids are received. An out of state contractor shall have Indiana State Gross Income Tax withheld by Allen County, unless he can show proof that he is registered with the Indiana Secretary of State to conduct business in the State of Indiana. No fee will be charged for permits required and issued by the Allen County Highway Department for work done on any Board of Commissioners of the County of Allen, Indiana owned property. The Board of Commissioners of the County of Allen, Indiana reserves the right to reject any and all bids for failure to comply with applicable laws and/or with the Notice to Bidders and reserves the right to waive irregularities.
