Demolition, site work and addition to an educational facility in Trafalgar, Indiana. Completed plans call for the addition of a educational facility; for the demolition of a educational facility; and for site work for a educational facility.

Work includes additions to the existing Indian Creek High School which includes a new Auditorium, Fieldhouse, Ballfields and Sitework. The completed pre-qualification forms with attachments shall be submitted to Misael Ramirez by no later than 5:00 PM Thursday, June 10, 2021. The project(s) will be constructed utilizing the Construction Manager as Constructor (CMc) delivery method for public work. The Construction Manager will contract with multiple First Tier Subcontractors for the Bid items listed below, with bids received on a lump sum basis for each bid item. Each proposal shall include all labor, equipment, and materials necessary to complete the project in strict accordance with the Construction Drawings, Project Schedule, Project Manual and Technical Specifications. The Construction Manager will receive sealed Bids for the following Bid Items of work: Bid Item #01: Site Excavation, Demo and Utility Work Bid Item #02: Asphalt Paving Work Bid Item #03: Site Fence and Gate Work Bid Item #04: Ballfield Grass Turf Work Bid Item #05: Ballfield Synthetic Turf Work Bid Item #06: Ballfield Lawn Irrigation Work Bid Item #07: Concrete Work Bid Item #08: PreCast Wall Panel Work Bid Item #09: Steel Work Bid Item #10: Masonry Work Bid Item #11: General Trades Work Bid Item #12: Roofing and Metal Panel Work Bid Item #13: Shingle Roofing Work Bid Item #14: Aluminum Windows, Curtainwall and Entrance Door Work Bid Item #15: Metal Stud, Drywall, Insulation, Acoustical Ceilings, Sound Panels and EIFS Work Bid Item #16A: Painting and Wallcovering Work Bid Item #16B: Staining of the Exterior Precast Wall Panels Work Bid Item #17: Sports Flooring Work Bid Item #18: Ceramic Tile Work Bid Item #19: Epoxy Terrazzo Work Bid Item #20: Epoxy Resinous Floor Work Bid Item #21: Carpet and Vinyl Base Work Bid Item #22: Locker Work Bid Item #23: Gymnasium Equipment Work Bid Item #24: Telescoping Stands Bleachers Work Bid Item #25: AuditoLEGALS rium Seating Work Bid Item #26: Theatrical Equipment Work Bid Item #27: Elevator Work Bid Item #28: Fire Protection Work Bid Item #29: Plumbing, Mechanical, HVAC Ductwork and Temperature Controls Work Bid Item #30: Electrical, Fire Alarm, AV and Technology Work SUBCONTRACTOR PREQUALIFICATION: All subcontractors must be pre-qualified prior to submission of the bid. Completed forms may be emailed to Misael Ramirez or a hard copy delivered to Hagermans office (10315 Allisonville Road) in a sealed envelope. Financial information will be kept confidential. BID SECURITY: Bidders are required to submit a Bid Security in the form of a Bid Bond in the amount of 10% of the Base Bid or certified check in the amount of 10% of the Base Bid. INDIANA FORM 96: Bidders are required to submit the Indiana State Board of Accounts Form 96 (revised 2013) including a signed Non-Collusion Affidavit which is part of Form 96. The Bid Form, Bid Security and Indiana Form 96 documents are required to be submitted with your Bid and shall be enclosed in a sealed envelope addressed to Hagerman, Inc. C/O Nineveh-Hensley-Jackson United School Corp. 802 South Indian Creek Drive, Trafalgar, IN 46181 Attn: Dr. Timothy Edsell. PERFORMANCE BOND AND PAYMENT BOND: A Performance Bond and Payment Bond in the amount of one hundred percent (100%) of the Contract Amount may be required of the successful bidder. Bidders shall provide a cost for these bonds on the bid form. SAFETY: All Contractors shall comply with OSHA and IOSHA rules and regulations regarding Safety for this project and All contractors shall abide by the 2020 Hagerman Safety manual. TAXES, PERMITS, INSPECTIONS: All Bids shall be submitted without inclusion in the bid price for the amounts, if any, of Indiana State Gross Retail and Use Tax (generally called the Sales Tax) for materials and properties that are to be purchased by the Bidder that will become a permanent part of the Project
