City of Lawrence, by and through its Board of Parks and Recreation ("City"), will receive sealed bids for the construction of eight (8) new pickleball courts at Lee Road Park, located at 6200 Lee Road, Indianapolis, IN 46236. Each bid must be submitted in a sealed envelope carrying the following information on its face: Vendor's name, address, and subject matter of the proposal: "Request for Sealed Bids- Lee Road Pickleball Courts." Each bid must comply with all conditions, requirements, and specifications contained in the Request for Sealed Bids, and must be accompanied by
(i) a completed Form 96 (including the non-collusion affidavit),
(ii) a financial statement,
(iii) information about the history of the vendor,
(iv) a minimum of three references,
(v) an overview statement of how the firm intends to perform the work and monitor the quality of the work in progress,
(vi) a list of equipment and materials for use, (vii) copies of quality assurance materials or plans,
(viii) a completed E-Verify affidavit, and
(ix) any and all other documents and information required by the Request for Sealed bids and/or statute. Bidders must be registered to perform work in the State of Indiana and shall include their license number on the proposal. Each bid shall be accompanied by a certified check or bond in the amount of 10% of the overall bid price. The bond or certified check shall be made payable to the City of Lawrence. All checks of unsuccessful bidders shall be returned upon selection of a successful bidder. The check of the successful bidder shall be held until delivery of a performance bond. The City reserves the right to reject any and all bids or any part thereof. The right is reserved to waive any formalities or informalities contained in any bid, and to award the contract to the lowest responsive and responsible bidder
