Project Description: Culvert Pipe Lining and Repair Upon Request Firms that wish to perform work on Division Purchase Order Contracts as either the prime contractor or as a subcontractor on the project must be prequalified for the type work they wish to perform. Firms that wish to bid on these projects as the prime contractor must be prequalified prior to submitting your bid. Firms that wish to perform as a subcontractor to the prime contractor must be prequalified prior to beginning work on the project. Any firm that wishes to bid as a prime contractor shall be prequalified as a Bidder or PO Prime Contractor prior to submitting a bid. The North Carolina Department of Transportation, in accordance with the provisions of Title VI of the Civil Rights of 1964 (78 Stat.252) and the Regulations of the Department of Transportation (49 C.F.R., Part 21), issued pursuant to such act, hereby notifies all bidders that it will affirmatively insure that the contract entered into pursuant to this notice will be awarded to the lowest responsible bidder without discrimination on the grounds of sex, race, color, or national origin. This renewable contract is an as-needed contract for culvert pipe lining and repair upon request on various interstate, primary, and secondary routes in Buncombe, Burke, Madison, McDowell, Mitchell, Rutherford, and Yancey Counties. The items and quantities included on the bid form are for bidding purposes only. This work shall consist of furnishing all labor, equipment, and materials to repair pipes using the cured in place pipe liner or the cementitious/geopolymer spray pipe liner method as required by the Engineer at locations determined by the Engineer. Work includes placement of the required lining type, filling voids around the pipe, headwall repair if required, dewatering the work site, cleaning the pipe, waste disposal, erosion control, and all incidental items necessary to complete the requested work as specified. Plans for dewatering at each site shall be approved by the Department prior to beginning work. Should the Contractor determine that a Right of Entry is needed at any work location, the Department will be responsible for its acquisition
