One (1) Original &Four (4) Copies Required. The Proposal Form shall also be accompanied by a properly executed non-collusion affidavit. Proposal is to be enclosed in a sealed envelope and distinctly show the name of the Bidder and marked on the outside: 2020 Capital Road Resurfacing Program. All bids must be presented in quadruplicate on the Proposal Forms provided and bids not on the Proposal Forms so provided may be rejected. Appropriately checked literature must accompany each copy of the proposal Form. Bids will be accepted by mail, or may be delivered or presented at the date and time set at the place herein listed above.
