This Project Involves the Rehabilitation of the State Red List Bridge (Br. No. 135/109) That Carries Nh 122 (Baboosic Lake Rd.) Over Nh 101 in Amherst. Bidders Should Act Promptly and Submit All Questions Pertaining to the Project in Writing to Jennifer Reczek at Jennifer.e.reczek@dot.nh.gov at Least Five (5) Business Days Before the Hour and Date Set for the Bid Opening. No Phone Calls or Faxes Will Be Accepted. All individuals, firms, partnerships or corporations intending to bid, must file a statement showing their qualifications with the Department of Transportation on forms prepared for that purpose at least ten (10) days prior to opening of bids. No authorization to bid will be granted to a prospective bidder not prequalified. Any information submitted as part of the Invitation to Bids may be subject to public disclosure under RSA 91-A. In addition, in accordance with RSA 9-F:1, any contract entered into as a result of the bid letting will be made accessible to the public online via the website Transparent NH . If contract price is $35,000 or more, the successful bidder will be required to furnish electronically, at the time of approval, a contract bond in the amount of One hundred (100) percent of their bid. All bidders will be required to execute a sworn statement pursuant to Section 112(c) of Title 23 USC, certifying that he or she has not either directly or indirectly, entered into any agreement, participated in any collusion, or otherwise taken any action in restraint of free competitive bidding in connection with such contract. Civil Rights Act of 1964,78 Stat. 252, USC 2005d to 2005d-4 and Title 49, Code of Federal Regulations, Department of Transportation, Subtitle A, Office of the Secretary, Part 21, Nondiscrimination in Federally-assisted programs of the Department of Transportation issued pursuant to such Act, hereby notifies all bidders that it will affirmatively insure that in any contract entered into pursuant to this advertisement, minority business enterprises will be afforded full opportunity to submit bids in response to this invitation and will not be discriminated against on the grounds of race, color or national origin in consideration for an award. The right is reserved to waive any informalities in or to reject any or all proposals. NHDOT - Amherst DESCRIPTION OF WORK This project rehabilitates the existing State Red List bridge (Br. No. 135/109) that carries NH 122 (Baboosic Lake Road) over NH 101 in the Town of Amherst. The bridge is located at the NH 122 and NH 101 interchange. The work begins on NH 122 approximately 180 feet east of the bridge, and continues 425 feet westerly across the bridge. The existing bridge, built in 1969, is a single-span steel girder bridge with concrete deck and a length of approximately 88 feet between abutments. The scope of work involves bridge rehabilitation including replacement of the concrete bridge deck, waterproofing membrane, and bituminous pavement; replacement of the expansion joint; replacement of the bridge rail and bridge approach rail; repairs to substructure concrete; and cleaning and painting of the structural steel and bridge shoes. Roadway work is limited to milling and bituminous overlay at the approaches to the bridge. The bridge rehabilitation work will be constructed in three phases while maintaining two lanes of traffic on NH 122 and full access to NH 101 interchange on-ramps and off-ramps. The painting operations will require lane shifts on NH 101, and some of the work will be performed at night requiring alternating one-way traffic with temporary traffic signals. A shoulder closure on NH 122 will be required for staging of equipment for painting operations.
