License Type Required: CA Class A or combination of Class C DBE Goal Percentage: 11% Project Duration: 115 Work Days Liquidated Damages: $4,800.00 Per Calendar Day Bid Bond Percentage: 10% MBE: 6.8% WBE: 6.9% DBE: 11% Reviewing published geologic maps, aerial photographs, project plans, in-house documents, and other literature pertaining to the site to aid in evaluating geologic conditions and hazards that may be present. A list of referenced materials is provided in Section 17.0 of this report. Performing a site reconnaissance to review project limits, determine drill rig access, and mark out exploratory boring locations for subsequent utility clearance. Notifying subscribing utility companies via Underground Service Alert (USA) a minimum of two working days (as required by law) prior to performing exploratory excavations at the site. Obtaining exploratory drilling permits from the Shasta County Environmental Health Division (EHD). Retaining the services of a California C57-licensed drilling subcontractor to perform exploratory borings. Performing four exploratory borings (Borings B1 through B4) to approximate depths ranging from 39 1/2 to 46 feet. Approximate exploration locations are shown on the Site Plan, Figure 2. Obtaining representative samples from the exploratory borings. Obtaining rock core samples of bedrock from select borings. Logging the borings in accordance with Caltrans guidelines. Upon completion, backfilling the explorations with neat cement grout per the Shasta County EHD permit requirements. Performing laboratory tests on selected samples to evaluate pertinent geotechnical parameters. Performing engineering analyses to develop geotechnical design criteria and evaluate existing site conditions. Preparing Logs of Test Boring (LOTBs) in general accordance with the Caltrans 2010 Soil and Rock Logging, Classification, and Presentation Manual. The LOTBs are presented in Appendix C. Preparing this FR in general accordance with Caltrans' Foundation Reports for Bridges, February 2017. This FR supersedes the Draft FRs dated November 2020 and March 2021 prepared by Geocon Consultants, Inc. for the project. The Whiskey Creek Road crossing of Whiskey Creek is located approximately 100 feet south of the Whiskey Creek Road intersection with Pioneer Road about 1 1/2 miles north of State Route (SR) 299. Existing Whiskey Creek Road at the creek location consists of two travel lanes (one in each direction) with unpaved shoulders of varying widths. At the crossing location, the existing culvert has failed and the surrounding roadway embankment fill has washed away. A replacement structure is required to reopen the road to two-way traffic. The creek has been spanned with a temporary Bailey Bridge approximately 70 feet in length and the roadway has been closed. The road crossing location on Whiskey Creek is near where the creek empties into Whiskeytown Lake. The crossing location is within the Whiskeytown National Recreation Area which is natural mountainous/forest land primarily used for recreational activities. Elevations used in this report are based on the current project plans that refer to the project datum of NGVD 29.
