This Request for Supplier Qualifications ("RFSQ") is an invitation by Hamilton Health Sciences Corporation ("HHSC") to prospective respondents to qualify in accordance with Evaluation of Responses (Part 2) for eligibility to provide Asbestos Abatement and Other Hazardous Materials Clean Up Services as further described in Appendix D RFSQ Particulars, Section A (the "Deliverables"). The intent of this request is to obtain services for asbestos abatement, mold, hazardous material remediation, repairs, and clean up at all HHSC sites, most specifically at McMaster University Medical Centre ("MUMC") on an on-going basis plus any project specific work before/during/after construction/renovations at the hospital sites. HHSC intends to develop a roster of two (2) pre-qualified respondents (Vendor of Record or "VOR") which will be utilized as required to meet the needs of the organization. The roster of pre-qualified respondents (VOR) is limited to asbestos abatement and other hazardous materials clean up services not exceeding $1,000,000.00. HHSC personnel perform regular inspections of asbestos-containing materials throughout all the hospital sites. When damages to asbestos-containing materials are reported, work orders are issued for asbestos clean up. In some cases as per specific project needs or emergency situations, asbestos abatement is also required. MUMC is the prime site for the above services, however this service may include other HHSC sites and McMaster University as requested. Quality standards, including whether specific expertise, is required for some or all of the Deliverables. This RFSQ is issued solely for the purpose of obtaining proposals and developing a VOR. Neither the issuance of this RFSQ nor the submission of a proposal implies any obligation by HHSC to enter into any agreement. The intent of this RFSQ is to identify those respondents capable of meeting HHSC's requirements and whom a final agreement may be negotiated.
