Morgan County, Georgia is seeking bids from qualified contractors to provide parking lot striping, hot pour crack seal and seal coating at Bill Wood Park The proposal shall be prepared simply and shall be straightforward and concise. The bid packet should include: o Bid Summary (attached in this packet) o Acknowledgement of bid amendments if any o A statement of qualifications to include copies of business and contractor licenses. o Contact information for at least 3 references for whom similar work has been completed within the last 12 months. o Detailed material and workmanship warranties o List of all subcontractors to be used. o Any other information deemed necessary by bidder to aid in award of purchase. o Completed E-Verify certification (attached in this packet) o Certificate of Insurance listing Morgan County as additional insured with minimum $1,000,000 general liability, $150,000 auto liability, and statutory worker's compensation liability o Current W-9 All questions about this RFP and submission requirements must be directed in writing to Ms. Lorrie Lavengood via email at llavengood@morgancountyga.gov Morgan County reserves the right to accept or reject any and all proposals or any parts of a proposal wherein its judgment, it will be in the best interest of the County; waive any technicalities/informalities in the RFP document and proposal process; and to qualify and award any or all of this contract in any manner in which Morgan County, acting in the sole and exclusive exercise of its discretion, deems to be in Morgan County's best interest.
