Include number of years your firm as been in business. 6. List your firm's present workload. Please specify the dollar value of PRESENT OUTSTANDING fee agreements. 7. Provide references. 8. Provide a brief description of your firm's equipment and facilities. Statements of qualifications should be transmitted to The Board of Pike County Commissioners c/o Denny T. As required by Ohio Revised Code, Section 153.65-71, responding firms will be evaluated and ranked in order based upon their qualifications. Following this evaluation, the Board of Pike County Commissioners will enter into contract negotiations with the most highly rated firm. The preliminary project description is as follows: Name of Project: Long Fork Road Slip Repair Project, PIK-CR26- 00.72 Location: Long Fork Road, Newton Township, Pike County, Ohio Description: Design Services utilizing the Geohazard Exploration Report as prepared by Terracon for this project. If subconsultants are to be utilized by the Prime Consultant, the Prime Consultant is required to take at least the following five affirmative action steps: 1. Place qualified small and minority businesses and women's business enterprises on solicitation lists. 2. Assure that small and minority businesses and women's business enterprises are solicited whenever they are potential sources. 3. Divide total requirements, when economically feasible, into smaller smaller tasks or quantities to permit maximum participation by small and minority businesses and women's business enterprises. 4. Establish delivery schedules, where the requirement permits, which encourage participation by small and minority businesses and women's business enterprises. 5. Use the services and assistance, as appropriate, of such organizations as the Small Business Administration (SBA) and Minority Business Development Agency of the Department of Commerce (DOC).
