Association with other individuals or firms having appropriate professional expertise is acceptable. The University will enter into a single agreement with the successful Contractor. A Bid Bond in the amount of five percent (5%) of the FLCC, made payable to the University of Utah shall accompany the cost proposal. Effective September 10, 2018, the University has retired the previous state mandated Surety Bond Forms and is now requiring the use of the AIA Bid Bond Form A310-2010 and Performance and Payment Bond Form A312-2010. The University of Utah reserves the right to reject any or all proposals or to waive any formality or technicality in any proposal in the interest of the University. This project will relocate Cardiovascular Prep & Recovery to the shelled space of University Hospital, building 525, level 4 Infill. The space left vacant will be remodeled into Inpatient Beds for CVMU. Phase 1: Provide new construction in existing shelled space including: (10) Prep & Recovery Bays including (3) High Acuity Bays and (1) Isolation Bay (room); (3) Recliner Bays; Nutrition/Supply Room; Soiled Hold; Clean Supply; (2) Toilet Rooms; Nurse Station & Medication Room. Each room or space will be fitted with required finishes, equipment, furniture, cabinets, power, data, mechanical, and plumbing. Gross Area is 2,865 SF Phase 2: Backfill Remodel: After the department moves into the new space, selective demolition will start on the old cardiac prep and recovery space. The remodel shall include (5) Inpatient Beds; (5) Inpatient Toilet, Shower rooms and a waiting area. Each room will be fitted with required finishes, equipment, furniture, cabinets, power, data, mechanical, and plumbing. Gross Area is 2,050 SF
