Sealed proposals for the following work will be received by the: CITY OF MADISONVILLE AND HOPKINS COUNTY FISCAL COURT HOPKINS COUNTY, KENTUCKY in the manner and on the date herein specified for the furnishing of all labor, materials, supplies, tools, services, etc., necessary for MADISONVILLE / HOPKINS COUNTY SPORTSPLEX BID PACKAGE I - ROUGH GRADING HOPKINS COUNTY, KENTUCKY as set forth in the specifications and drawings prepared by Sherman Carter Barnhart Architects, 2405 Harrodsburg Road, Lexington, Kentucky 40504 Project No. 221-170 Each proposal shall be submitted on forms obtained from the project manual, Volume 1, Division 0, "Procurement and Contracting Requirements": Form of Proposal and Form of Proposal Attachment. Proposals shall be enclosed in a sealed envelope with the following information on the outside: "BID DOCUMENTS" MADISONVILLE / HOPKINS COUNTY SPORTSPLEX BID PACKAGE I - ROUGH GRADING HOPKINS COUNTY, KENTUCKY DECEMBER 7, 2021 One copy of the completed Form of Proposal, with Form of Proposal attachments, will be required from each bidder. Forms of Proposal shall be completely filled out and submitted prior to the time of bids. Corrections to information on the Bid Form must be initialed to be valid. Forms of Proposal not completely or correctly filled out will not be considered. No proposal shall be withdrawn for a period of sixty (60) days after the date of bid opening. The successful bidder will be required to provide a Performance and Payment Bond, written on AIA Document A312, in the amount of 100% of the contract sum. The owner's desire to waive irregularities and informalities as to a bid shall be reviewed and final judgment made prior to approval of a contract. A Bid may not be modified, withdrawn, or cancelled by the Bidder for a period of sixty (60) days following the time and date designated for receipt of Bids and Bidder so agrees in submitting his Bid.
