Lead Hazard Reduction & Healthy Homes Intervention 109 Easton AV Clarification Deadline 12/1/2021 2:00 Pm The Waterbury Healthy Homes Program is undertaking a Healthy Homes initiative project at 109 Easton Avenue located in Waterbury, Connecticut. The Site building consists of a three (3)-family residential building. A Healthy Homes Inspection was performed in accordance with the U.S. Department of Housing and Urban (HUD) Office of Healthy Homes and Lead Hazard Control. The inspection was performed by the Waterbury Healthy Homes Program personnel and utilized the Healthy Homes Rating Tool (HHRT) provided in the Healthy Homes Rating System (HHRS). The HHRS utilizes a rating scale concerning the following factors: Class of Harms (Severity), the Likelihood that the harm will occur in the specific residence and Spread of Harms (how likely it will occur at certain severities). The combination of these factors is summarized with a Hazard Score. The Waterbury Healthy Homes program personnel develop the Hazard Score. Eagle Environmental, Inc. (Eagle) has developed a Healthy Homes Scope of Work based off the findings of the Healthy Homes Inspection for the Waterbury Healthy Homes Program. The Scope of work includes the Inspector's findings and any findings identified by the project designer during their visit. The Waterbury Healthy Homes Program has provided Eagle with the summary of their findings identifying the healthy home hazards in each dwelling unit, associated common areas and the exteriors of the Site building. The Contractor performing the work specified under this Section shall be a State of Connecticut licensed Home Improvement Contractor (HIC) and shall appropriate State and Federal Certifications and Licenses where necessary (i.e. USEPA RRP). All workers and supervisors conducting work under this section shall be appropriately trained, licensed or certified by the State of Connecticut by the appropriate authorities where necessary, including all hazardous materials licenses and certifications when any hazardous material will be affected. All sub-contractors must be pre-approved by the Waterbury Healthy Homes program. All work specified in the Healthy Homes Scope of Work shall be performed in accordance with local building codes. Additionally, work shall be performed in accordance with the U.S Department of Housing and Urban Development (HUD), The United States Environmental Protection Agency (USEPA), The State of Connecticut Department ofPublic Health (DPH) Lead Poisoning Prevention and Control Regulations, The State of Connecticut Department of Energy and Environmental Protection (DEEP) Hazardous Waste Disposal regulations and the Department of Labor's Occupational Safety and Health Administration (OSHA) regulations. SCOPE OF WORK The contractor is only to provide pricing for those items identified on the attached Scope ofWork. The Scope ofwork shall be prioritized by the Waterbury Healthy Homes Program, it maybe that not all line items will be performed in the Scope of Work. The Waterbury Healthy Homes Program has the option ofselecting some or all of the items in the scope of work. There is a separate Healthy Homes Scope of Work table provided in this plan identifying items that are the Home/Building Owner's responsibilities. This Table does not include items that the Waterbury Healthy Homes Program excluded from the contactor's Scope of Work. Refer to the Contractor's Scope of Work for excluded items that will ultimately become the Home/Building Owner's responsibilities.
