The proposed work includes collecting, transporting, and disposing of municipal sewage plant sludge from the Townships Sewage Treatment Plant located at 18 Fostertown Road, Medford Township, New Jersey. Please submit your Letter of Federal Approval or your New Jersey Certificate of Employee Information Report with this bid. If you do not have either document, you will be required to complete and submit Affirmative Action Employee Information Report (AA302). Form (AA302) is available from the Township of Medford. Each bid must be accompanied by a Non-Collusion Affidavit, an Ownership Disclosure Form, a Plant, Equipment and Experience Statement and a proposal guaranty in the form of a certified check or bid bond in an amount not less than ten percent of the Total Bid Price Bid in the proposal; except that when the Total Price Bid is $200,000.00 or more, the amount of the proposal guaranty shall be $20,000.00. The proposal guaranty must be in the form of a bid bond, or certified check, or cashiers check or a combination thereof, and must be drawn to the order of the Township of Medford, New Jersey and must be delivered at the place and by the hour above-named. Proposals received after the designated time shall be deemed unresponsive and shall not be opened. All proposals submitted by corporations or partnerships shall list names of stockholders in accordance with P.L. 1977, C.33. Bidders are required to comply with the requirements of P.L. 1975, C.127 which is an act relating to affirmative action in relation to discrimination in connection with certain public contracts; and shall pay prevailing wage rates in accordance with P.L. 1963, C.150. Bidders are required to comply with the requirements of N.J.S.A. 10:5-31 et seq and N.J.A.C. 17:27. The Township of Medford reserves the right to reject any or all bids and to waive any insignificant defect or informality in any bid should it be in the interest of the Township to do so. The payment for the contract documents is not refundable. If contract documents are to be mailed, there is an additional $15.00 fee for postage Proposals must be made on the standard Proposal Form, in the manner designated therein and required by the Specification.
