The City of Cleveland Mayor's Office of Capital Projects (MOCAP) requests a proposal for furnishing and installation of a 25' steel Gazebo at Mile Heights Park in Ward 1. Please find the attached gazebo plan, detailed drawings and schedule of items that depict the general location and requirements. Additionally, the following requirements for implementation of the Gazebo installation should be included as part of your proposal: -Furnish and install 25' steel gazebo. -Fine grading and seeding of disturbed areas. -Clean-up of the installation area(s), removal of excess soil and debris and repair of lawn area to match existing conditions. 1 . Included in this Item, all costs generally attributable to general conditions and limited to: equipment, field superintendence, protection of area, temporary utilities, contractor laydown area/staging, office costs, safety signage, security, trash removal, porta pit, mobilization, phasing, bonding and profits, etc., complete. 2 . Furnish and Install 25' Steel Gazebo to match basis of design ICON HX25M-P6 w/ Corregated steel roof painted both sides Foundations to be installed by separate contractor - provide bolts 3 . Furnish and Install 4' topsoil, Fine Grade & Seed disturbed areas Service Dates : From 2021-11-01 To 2022-01-31
