Project Overview This scope of work is for an engineering assessment of KY00029 Clements Lake Dam, located on the campus of Morehead State University. The project includes a review of available information for the dam, in-depth engineering inspection of the dam and identification of deficiencies, hydraulic and hydrologic (H&H) analyses, geotechnical exploration and laboratory testing, and slope stability analyses, and development of remediation alternatives for any deficiencies identified. Other tasks include twodimensional breach analysis and development of an Emergency Action Plan and Probable Failure Modes Analysis (PFMA). Primary Project Objectives o Review available information and identify critical data gaps. o Provide a better understanding of the condition of the dam and identify deficiencies by performing an in-depth inspection of the dam and an updated H&H analysis, geotechnical exploration, and geotechnical analyses. o Determine risks posed by the dam by performing a breach analysis and Potential Failure Mode Analysis (PFMA) o Provide risk mitigation by developing and coordinating an Emergency Action Plan. o Develop conceptual alternatives to bring the dam into compliance with state dam safety standards. Project Tasks o Data Collection and Review - Review files associated with the dam. KDOW will provide access to the files for the dam. Coordinate with Morehead State University for information. Provide a summary of identified data gaps that will require attention in subsequent tasks. o In-Depth Field Inspection - Perform a detailed engineering inspection of the dam to determine the current condition and identify any deficiencies. Provide a stamped inspection report to KDOW detailing observations made and deficiencies identified. o Survey - Perform a topographic survey of the dam and a bathymetric survey of the reservoir. Provide information in digital format, including a 3-dimensional terrain surface and stage-areastorage data for the reservoir. Survey downstream hydraulic structures and lowest floor elevations of all structures potentially impacted by a breach of the dam to support the breach analysis task. o Hydrologic & Hydraulic Analyses - Perform H&H analyses to determine the current hydraulic characteristics and capacity of the dam. Compare results to regulatory requirements and provide report. o Geotechnical Exploration - Perform a geotechnical exploration and laboratory testing program to gather baseline information about the dam embankment and foundation to support geotechnical stability analysis. Develop a geotechnical exploration plan and boring plan for KDOW review and approval. Install observation wells and pressure transducers (Solinst Levellogger 5 with L5 Direct Read Cables) to observe and record internal water levels. Submit a report of findings. o Geotechnical Stability Analysis - Perform an analysis of the structural stability of the structure including the following loading cases: long-term steady-state seepage under normal operating conditions, rapid drawdown (upstream face only), and seismic loading. o Potential Failure Mode Analysis (PFMA) - Perform a Potential Failure Mode Analysis and provide a report of findings. o Breach Analysis - Perform a two-dimensional unsteady flow breach analysis and inundation mapping. o Emergency Action Plan - Develop an Emergency Action Plan following the guidelines of FEMA P64. Coordinate with stakeholders and conduct a table top exercise review. o Develop Conceptual Remediation Alternatives - Based on the assessment develop conceptual repair alternatives to address unacceptable risks at the dam related to deficiencies identified. Include an opinion of probable construction costs. o Reporting - All results of the above assessments are to be provided to KDOW in report form with all digital supporting data. Deliver report(s) as drafts for KDOW review and comment prior to issuing final report(s). o Project Management - Provide project management for the project including the following: monthly progress meetings, kick-off meeting with KDOW at the start of the project, meetings to review the results of the analyses outlined above, meeting to review recommendation and identify conceptual alternatives, EAP tabletop exercise and stakeholder meeting and final project close out meeting.
