Demolition and renovation of a mixed-use development in Grand Island, Nebraska. Completed plans call for the renovation of a medical facility.
The project area entails all floors. The project will include demo of existing hardware and wiring and installation of new equipment and wiring. The contractor shall Provide all labor, materials, tools, equipment, and construction services necessary to complete the construction of the project described herein. Work includes general construction, electrical and other trades as required to provide a complete and finished product. DESCRIPTION OF WORK a) Contractor shall assume all areas of the campus are and must remain fully operational. (Except as noted in 2.h, below.) b) Dedicated Prime Contractor Superintendent must be on site while work is being performed and must be OSHA 30 hour certified. c) All work must be contained within the approved construction limits and must maintain a clean working site. d) Contractor shall notify COR at least 48 hours prior to any significant construction activity. e) Contractor shall install temporary environmental controls prior to construction. f) Contractor shall repair any areas damaged by installation, to include landscaping. g) Contractor shall remove all debris from VA property. Construction and demolition non-hazardous solid waste report is required to be submitted quarterly. h) Contractor shall provide a proposed schedule for conducting all tasks for completing all items in this SOW. o Contractor shall especially coordinate with COR, VA Police, and/or M&O during times even the present system or portions may not operate, so alternate notification processes can be implemented. i) Contractor shall provide an Accident Prevention Plan (APP) no later than 5 calendar days prior to preconstruction meeting. j) Contractor shall provide an Activity Hazard Analyses (AHA) at least 5 calendar days prior to the start of each phase. k) Contractor shall comply with all applicable Federal, State and Local codes including but not limited to: o Energy Policy Act 2005 o Energy Independence and Security Act 2007 o All State and Local Jurisdictions enforcing Federal Law 1.06 o International Building Code, all chapters o OSHA Standards o NFPA 101 Life Safety Standards o VA Accessibility Standards l) Project Meeting Management. The Contractor shall take minutes of all project meetings. The format must be acceptable to the CO and COR. Minutes must include the meeting date and time; agenda; meeting location; list of participants with their affiliations, and telephone numbers; be fully descriptive of issues, problems and decisions made; and action items with names of responsible parties and deadlines. In addition, the minutes must include, as attachments, all exhibits and references distributed as handout materials, unless the materials are part of a formal submission. The Contractor shall provide, via email transmission, complete copies of all minutes to CO and all attendees within 48 hours of the next meeting. All challenges to the minutes shall be reflected in a revised version prepared by the Contractor. Copies of updated minutes shall be distributed to the CO and all attendees within 24 hours after change notice. m) Picture taking (if required) of existing facilities require written notification to be made to the Contracting Officer Representative (COR) no later than seven (7) days before the requested date to take pictures. Pictures shall be limited to areas approved by the COR, and shall not include pictures of VApatients,staff, or the public ACG Project Number 18-030
