The Cuyahoga Metropolitan Housing Authority (CMHA) is seeking quotes from qualified, licensed, and insured companies to provide all labor, materials, equipment, supplies, and services for High Rise Entrance Door Repairs. As-needed work shall be authorized by purchase orders on a project-by-project basis. Each purchase order shall reference a scope of work for that specific project and include a fixed fee and completion date The work to be performed under the contract is subject to the requirements of Section 3 of the Housing and Urban Development Act of 1968, as amended. CMHA is a Political Subdivision of the State of Ohio and is exempt from all taxes. CMHA seeks to establish pricing with a company who can repair, remove, and/or replace various entry doors at our High-Rise Buildings at CMHA properties on an as-needed basis. CMHA has 33 High-Rise Buildings. Work shall consist of, but not be limited to: 1. Entrance Door All questions regarding the RFQ and requirements must be submitted in writing to gilbertp@cmha.net prior to 4:00 pm on Thursday, January 13, 2022, and will be answered as addendum and posted electronically This request for quote is subject to the requirements of Section 3 of the Housing and Urban Development Act of 1968, as amended. Preferences may be given in accordance with 24 CFR 135 and the CMHA purchasing policy. There is a 30% goal for hiring Section 3 residents on any contract resulting from this RFQ. Any Section 3, MBE, or FBE qualified vendor should state specifically such status. Quotes from minority and female business enterprises are encouraged. For any questions related to economic inclusion, please contact Lydia Sanders, Section 3 Coordinator, at SandersL@cmha.net.\ The Authority will not execute a contract on the successful bidder's form of contract. See Attachment D for Small Purchase General Terms and Conditions. By submitting a response, the successful bidder agrees to the terms in the RFQ and the attachments. All contract documents, including any contractor supplied agreements shall be reviewed by the Contracting Officer prior to execution. Services shall not be performed for CMHA without an authorizing Purchase Order received by the Contractor by e-mail. Please note that contracts are limited to $50,000.00 over the term of the contract. The term shall not exceed one year. Invoices or vouchers for payment shall be certified by an approved and responsible official of the Contractor's organization. Each invoice or voucher shall, at a minimum, be supported by a summary of the cumulative costs, a description of the service provided and the date of the performance of service. Each Invoice or Voucher Must Also Show the Contract/ Purchase Order Number.
