Site work and renovation of an educational facility in Evansville, Indiana. Completed plans call for the renovation of a educational facility; and for site work for a educational facility.

All proposals shall be made out and submitted on a bid proposal form attached to a current General Form No 96 a bid form prescribed by the State Board of Accounts. Any bid that exceeds a sum of $5,000.00 must be accompanied by a Contractor's Financial Statement attached to the General Form 96 as prescribed by the State Board of Accounts. One General Form No. 96 will be required from any one contractor for one or more bids. All proposals shall be firm without escalator clauses. The proposal must be accompanied by a certified check made payable to Evansville Vanderburgh School Corporation in an amount equal to not less than five percent (5%) of the base bid or an acceptable bidder's surety bond made payable to Evansville Vanderburgh School Corporation, in a SPECIFIC AMOUNT of not less than five percent (5%) of the base bid, executed by a surety company authorized to do business in the State of Indiana. The certified check or bidder's bond shall be a guarantee that said bidder will, if the contract is awarded to him, execute within ten (10) days from the acceptance of his proposal a contract for the work bid upon, and furnish acceptable surety bonds as set forth in the specifications. However, if the contractor has not executed the contract within the ten (10) day period, or has failed to furnish acceptable surety bonds as indicated, the Board of School Trustees reserves the right to award the contract to the next higher or best bidder. All local licensing requirements must be met prior to submitting bid (Building Commissioner #812- 436-7867). All General Contractors must be certified for Institutional Buildings (154201) with the Indiana Department of Administration Public Works Board. Inclusion of drug policy is mandatory with proposal. All persons intending to submit a bid with EVSC must comply with MBE/WBE utilization and E-Verify policies and procedures as outlined in the specifications. No bidder may withdraw his bid for a period of ninety (90) days after the date set for the opening of bids The Board of School Trustees of the Evansville Vanderburgh School Corporation reserves the right to reject any and all bids and to waive any defects or informalities if deemed by it to be for the best interest of said Board. Each bidder shall visit the site of the proposed work and fully acquaint himself with conditions affecting the work under this contract and any possible ramifications due to the presence of asbestos-containing materials (ACM) or lead-based paint. (See Paragraph A25 below.) Bidders shall thoroughly examine and be familiar with the drawings and specifications. The failure or omission of any bidder to examine any form, addendum, other documents or failure to visit the site and acquaint himself with conditions there existing shall in no way relieve any bidder from any obligation with respect to his bid or to the contract. If any person contemplating the submitting of a bid for the proposed contract is in doubt as to the true meaning of any part of the plans and/or specifications, he may submit to the Architect a written request for an interpretation thereof. The person submitting the request will be responsible for its prompt and actual delivery. Any interpretation of such plans and/or specifications will be made only by addendum, duly issued, and a copy of such addendum will be mailed or delivered to each person or bidder receiving a set of plans and specifications from the Architect. Neither the Owner nor the Architect will be responsible for any other interpretations or explanations of such plans and/or specifications which anyone presumes to make on behalf of the Owner prior to the ultimate date set for the receipt of bids. Attention is called to the fact that firm bids are requested without escalator clause or clauses. Bids shall be submitted in sealed envelopes, identified on the exterior as to the bidder's name and clearly listing the project bid title and category bidding on. One prime bid per envelope.Each bid submitted in response hereto shall be for a precisely stated amount (expressed in dollars and cents) representing the total charge that would be made against the Board by the bidder for the bidder's complete performance of the contract contemplated hereby, which amount is hereinafter referred to as "the bid price." A bid shall not include any provision to the effect that the bidder will charge to, or collect or receive from, or any other way impose upon, the Board in addition to the bid price, any amount in respect of any tax. The Owner is exempt from payment of the Indiana Sales Tax as evidenced by Exemption Certificate No. 701603-04 issued by the Indiana Sales Tax Division of the State of Indiana. The contractor will obtain any further or additional certificates as may be required to effect the exemption. It Is the Responsibility of the Contractor to Be Certain That All Documentation Is Included With Their Bid. Bids, which are not signed by the individual making them, shall have attached thereto a power of attorney evidencing authority to sign the proposal in the name of the person for whom it is signed. Bids, which are signed for a partnership, shall be signed by all of the partners or by an attorney-in-fact. If signed by an attorney-in-fact, there shall be attached to the proposal a power of attorney evidencing authority to sign the proposal executed by the partners. Bids, which are signed by a corporation, shall have the correct corporate name thereon, and the signature of the president or other authorized officers of the corporation manually written below the corporate name. Such proposal shall also bear the attesting signature of the corporation and the impression of the corporate seal. Any bid in the amount of FIVE THOUSAND DOLLARS ($5,000.00) or more must be accompanied by a Contractor's Financial Statement attached to a current General Form No. 96 as prescribed by the State Board of Accounts of the State of Indiana. No bid can legally be accepted unless accompanied by this form, properly filled out and executed. Surety Bonds (separate performance and payment bonds) shall be furnished by each contractor to whom a contract is awarded by the Owner, with the premium for such bonds assumed by the contractor Such Surety Bonds shall be: a. Furnished prior to the start of any work at the site. b. Issued in accordance with the governing state laws applicable thereto by a reputable corporate surety approved by the Owner c. Issued in a penal sum at not less than 100% each conditioned upon the proper and full performance of the contract, including, but not by way of limitation, the plans and specifications or any modifications or supplements thereof. The following insurance shall be secured and paid for by each contractor, and in the event of subcontractors who are not covered by the policy of the PRIME contractor, it shall also be secured and paid for by each such subcontractor, all in Accordance With the Statutes of the State of Indiana. Workmen's Compensation, Employer's Liability and Occupational Disease Insurance: To be provided for the protection of all employees engaged under each PRIME contract to the extent provided by the Workmen's Compensation Act and the Occupational Disease Act of the State of Indiana The Department of Buildings of the City of Evansville will issue one building permit to the General Contractor, upon payment to the Department a fee of TEN DOLLARS ($10.00) by the General Contractor
