Renovation of a medical facility in Plymouth, Wisconsin. Completed plans call for the renovation of a 825-square-foot medical facility.
A single contract will be awarded for providing all permits, labor, equipment, and materials required to furnish and install all required materials and labor to complete all of the work specified in the bid documents including removal and lawful disposal of the items being replaced. All bids received must remain in effect for not less than 90 days following the due date. Attention is called to the fact that this project is funded through a Community Development Block Grant-Public Facilities grant and the Contractor as well as any Sub-Contractors will be required to pay not less than the Prevailing wages rates, as published by the United States Department of Labor and System for Award Management. A copy of determination # WI 20210016 Mod # 4 effective December 4, 2021 is included in the bid documents. The bidder's attention is also called to the Notice for the Requirement of Affirmative Action to insure Equal Employment Opportunity for employment of women and minorities in the project area. In addition, it is the policy for this project that minority business enterprises shall have the maximum opportunity to participate in the performance of contracts financed in whole or in part with Federal funding under this agreement. Attention of bidders is also called to the fact that the successful bidder must insure that employees and applicants for employment are not discriminated against because of race, color, sex, religion or national origin. Federal requirements for equal opportunity (Executive Order 11246, Segregated Facilities section 3 and section 109 and the conditions of employment and wage rates to be paid under the contract. The project is being funded in part through a grant from the State of WI, Department of Administration, Community Development Block Grant-Public Facilities and the United States Department of Housing and Urban Community Development Block Grant Program. In order to be considered, all bidders must have a representative in attendance at the pre-bid conference. The successful contractor shall maintain, and furnish a certificate of insurance to the Purchasing Agent for Worker's Compensation Insurance for all workers on this contract, as well as Public Liability and Property Damage Insurance, including Contractors Contingent and Protective Insurance, as will protect him and any subcontractor performing work covered by this contract, from claims for damages for personal injury, including accidental death as well as claims for property damages which may arise from operations under this contract. During the term of the contract, the General Contractor as well as any Sub-Contractors will be expected to work closely with the County's third-party provider of prevailing wage compliance monitoring, The Bay Lakes Regional Planning Commission is the provider of these services. It is Sheboygan County/ Rocky Knoll policy that all employees of Contractors and his or her sub-contractor that will be performing work within the facility will submit to and successfully pass a criminal background check prior to being allowed access to the facility. The successful contractor will be furnished with the appropriate forms and will be required to submit same to the county no less than 72 hours prior to mobilization. The County shall retain the right to refuse access to any person whom, in the opinion of Sheboygan County, is unable to successfully pass the background check. The contract document will accompany a Sheboygan County Purchase Order referencing the terms and conditions of the bid documents. All bids received shall remain in force for not less than (120) One-Hundred Twenty days following the bid due date. The County of Sheboygan is exempt from Federal Excise Tax and State Sales Tax. Bids shall be made exclusive of these taxes. Certification will be furnished to the successful bidder upon request. Sheboygan County reserves the right to reject any or all bids, cancel this solicitation, to waive informalities in the bidding process, or to accept any bid considered most advantageous to the County of Sheboygan. The Owner anticipates commencement of work in 2022 and completion in 2022 The successful bidder must agree to commence work on or before a date to be specified in a written Notice to Proceed and to fully complete the work within 150 consecutive calendar days thereafter. A. Summary Project Description: Renovation of existing resident & living rooms into classroom facility. Removal of plumbing fixtures, flooring & base, toilet room door, and window treatments. Installation of solid surface material in toilet room. New wall finish and paint throughout. Removal of furniture & repair to ceiling. Provide new furniture.. B. Contract Scope: Construction, demolition, renovation, and facility operations during occupancy. This project involves conversion of two Resident rooms and a common area (2002 Construction) into a classroom suitable for use by both Nursing Home Staff and Residents. Included is selective demolition, plumbing, electrical, general construction, Heating Ventilating and Air Conditioning. Also included are casework, floorcoverings, ceiling work and window coverings. *The list of planholders and/or interested bidders is not being tracked by the stakeholders involved with this project.
