DESCRIPTION OF WORK: This work shall include, but is not limited to: Asphalt and Concrete Milling, Patching, and Placement of HMA Wedge and Level and HMA Surface of 165th St. from Columbia Ave. to Summer St. in the City of Hammond, Lake County, Indiana. COMPLETION DATE: This work shall be completed by September 1st, 2022 All Bidders must be properly licensed in the City of Hammond at the time bids are received. Contractors may contact the Hammond Building Inspections Department to inquire on General Contractor and Sub-contractors licensing and requirements, at 219.853.6316 or the Hammond website, Inspections Department for applications. Please note that the Hammond Contracting Board only meets once per month, at the first Monday of every month at 6:00 pm in the Inspection Department Conference Room. The deadline for submitting an application and required information for a Hammond License is one week prior to the meeting. All bidders are advised that they must comply with the requirements in Ordinance No 9519 relating to responsible bidding practices. A copy of this ordinance is included in the Bid Documents No bidder may withdraw his proposal for a period of 60 days following the date set for receiving bids. The City of Hammond reserves the right to retain any and all bids for a period of not more than 60 days, and said bids shall remain in full force and effect during said time. The City of Hammond reserves the right to waive informalities and to award the contract to any Bidder all to the advantage of the City of Hammond or to reject all bids. No Pre-Bid Conference will be held. BID SECURITY: Each bid shall be accompanied by an acceptable certified check or cashier's check made payable to the City of Hammond or an acceptable Bid Bond in an amount not less than five percent (5%) of the total bid price executed by an incorporated surety company in good standing and qualified to do business in the State of Indiana. BONDS: A performance bond and payment bond each in the amount of one hundred percent (100%) of the contract price will be required. INDIANA REQUIREMENT: Standard Indiana Form 96 (Revised 2009) which includes non-collusion affidavit will be required. Contractors submitting a bid for City construction projects must subscribe to the Indiana Plan and a joint apprenticeship and training program for that project if their bid is accepted and provide proof of same before any payments are made. Each bidder must ensure that all employees and applicants for employment are not discriminated against because of their race, creed, color, sex or national origin and each bidder must conform with the antiiscrimination provisions of IC 5-16-6. An Affirmative Action Statement must be submitted at time of bid. Each bidder shall include in their Bid Packet their written Drug Testing Plan that covers all employees of the Bidder who will perform work on the project that meets the requirements set forth in IC-4-13-18-5 or IC4-13-18-6. Each prime contractor submitting a bid for this contract must be prequalified by INDOT Each successful bidder and any and all levels of subcontractors, regardless of tier, as a condition of being awarded a contract or subcontract, will agree to abide by the provisions of the 2021 Hammond Community Crossing Project: Asphalt and Concrete Milling, Patching, and Placement of HMA Wedge and Level and HMA Surface of 165th St. from Columbia Ave. to Summer St. Project Labor Agreement as executed by and between the Project Manager and/or Construction Manager authorized to negotiate and enter into such a Project Labor Agreement on behalf of the City of Hammond, Indiana (or a corporation created to benefit the said entity in construction, alteration, or repair) and the local labor unions representing experienced and skilled construction workers and will be bound by the provisions of that Agreement in the same manner as any other provisions of the contract."" Twenty five (25%) percent of the contractor's work force in each initial craft will be comprised of residents of the City of Hammond participating in the Indiana Plan, whenever possible.
