Refrigeration Equipment Preventative Maintenance The Pennsylvania Department of Human Services ("DHS") requires a Contractor to provide all labor, transportation, equipment, and tools necessary to service and repair Refrigeration Equipment at Clarks Summit State Hospital (the "Facility") located at 1451 Hillside Drive, Clark Summit, PA 18411. The selected Contractor shall be available to provide service seven days a week, 24 hours a day. Preventative Maintenance will be through annual and semi-annual visits as noted in Attachment A of the Statement of Work. The Contractor must also be available for Repairs on an on-call basis, if needed. 2 Years with option for 3 (one year renewals)
