Price Schedule Instructions/Notes: 1. Volumes may be subject to change with minimal notice. Contractor shall support Metropolitan's requirements whatever the actual volumes may be. 2. No guarantee as to minimum or maximum order quantities is given. Orders will be placed only as requirements arise. Evaluation will be on the projected quantities stipulated in the line items. 3. The prices quoted must include all costs, including freight to deliver product to Metropolitan facilities. Caustic Soda purchases are exempt from sales and use tax. No additional charges allowed, including but not limited to, fuel or energy surcharges. 4. FOB/Freight Terms are: FOB Destination, Freight Prepaid and Absorbed by Contractor. Recent Projects [Section V] - Found in this document and it will provide information on four [4] of Bidder's recent projects that are most similar to Metropolitan's RFB. 3. Respondent Questionnaire [Section VI] -Found in this documents and it will provide Metropolitan with information as to how the bidder meets the qualifications required in the bid. 4. List of Subcontractors [Section VII] - This worksheet is found in this document and it provides Metropolitan with information on the subcontractors that you will use (if any) for this contract. 5.
