The City of Keizer is requesting proposals from an individual or company for services described in the Scope of Services. 1. City to remove all furniture/equipment, if any without involvement of contractor. 2. Remove and dispose of existing carpet tile. It is recommended that Contractor recycle the existing carpet tile utilizing a program/company designed to recycle carpet tiles. 3. Repair wall damage as a result of carpet removal. 4. Prepare floor for new tile carpet. 5. Provide adhesive. 6. Provide Shaw Contract Think Tile Color Achieve 86760 carpet tiles (24" x 24"). 7. Repair wood trim to match current wood trim damaged as a result of carpet removal or installation. 8. Install carpet tiles in the civic center lobby to the slate floor (approximately 40' x 30' - 1200 square feet). 9. Provide and install any transition strip(s) necessary between carpet tile and other surfaces. 10. The project shall be installed and finished pursuant to any and all standards and requirements of the manufacturer as of the date of the contract to ensure warranty. The City is dependent on Contractor's expertise and experience in this regard. 11. Properly dispose of all waste material from project. 12. Comply with appropriate safety measures. 13. Obtain satisfactory final inspection from the City of Keizer.
